The Lead Practice Nurse supervises Practice Nurses and other office personnel along with performing duties as a Practice Nurse including providing telehealth services from a local office and handling live calls for all patients of multiple CNEMG Primary Care Physician Practice locations. Through a centralized call-center, this position provides telephonic triage and document review services that require clinical judgment and interpretation. Acting as a liaison between patients and their care teams, the Practice Nurse is independently responsible for providing health guidance for minor acute illnesses and significant chronic illness care and management. As a member of a team of nurses, this position will also assist in managing the triage message pool for the Primary Care call-center.

Duties and Responsibilities:

Assist in the supervision of work and performance of Practice Nurses.

  • Supervises office personnel and addresses performance issues with Manager.
  • Assists in addressing and resolving patient concerns and complaints.
  • Monitor vacation and sick time; arrange coverage as appropriate.
  • May participate with interviewing candidates, orienting and then training new staff.
  • Administer competencies for new staff.
  • Participates in organizational and unit-based quality improvement initiatives and compliance readiness.
  • Monitor office and medical supply inventory.
  • Provide input into the annual evaluations of the office personnel.
  • Monitor and provide support for users in multiple computer applications.
  • Provide recommendations to update and maintain department procedures.
  • Perform duties as a Practice Registered Nurse.

Assessing patient symptoms over the phone to determine patient condition and the most appropriate place/mode of service for their condition.

Answering clinical questions for patients and their caregivers.

Delivering test results and educating patients and their caregivers about the plan of care determined by the clinician.

Serves as a patient educator teaching patients and their caregivers about their medications, symptom management, preventative health measures, and other topics related to their health.

Independently provides health guidance for minor acute illnesses as well as chronic illness care and management.

Operating under standing orders or approved protocols provides assessment, management, and treatment, including prescribed medications, to patients presenting with well-defined symptoms or conditions

Conducts medication reconciliation as appropriate and communicates any need for change or adjustment to care team and clinicians. Provides support to patients to enhance medication adherence.

Acts as a liaison helping to coordinate Care for patients and their care teams including but not limited to Visiting nurses, Nurse care managers, Community health workers, social workers, etc.

Serves as a resource to lower licensed or unlicensed staff with clinical questions and assistance with clinical tasks.

Performs all other related duties as assigned.

Requirements:

Associate's Degree Required; Bachelor's Degree Preferred.
