Lead Advanced Nurse Practitioner (Long Term Conditions)
Birtley Medical Group is looking to recruit an experienced Lead Advanced Nurse Practitioner/Specialist Nurse (Long Term Conditions) to join our large friendly practice in Gateshead. The post holder will work closely with our Clinical Pharmacists focusing on long term conditions and also providing clinical leadership to our large nursing team. 24-30 hours per week, salary depending on experience.
Main duties of the job
The Lead Nurse will be an experienced nurse who, acting within their professional boundaries, will provide care for patients presenting at the practice, to include clinical assessment, treatment and evaluation of care. They will demonstrate safe clinical decision-making and expert care, including assessment and diagnostic skills, for patients within the general practice.
They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures and providing clinical leadership to the practice nursing team. They will ensure all competency and training requirements of the nursing team are met to ensure the safe delivery of care and play an active part in ensuring the readiness of the Practice for a CQC inspection. They will be supported by GP partners who will provide clinical supervision.

Job responsibilities
Clinical practice
- Assess patients presenting for review of chronic disease conditions with a focus on, but not limited to diabetes.
- Work with the practice team to foster care and support planning with people with long term conditions and increase patients' participation in their healthcare.
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
- Manage chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
- Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols and formularies.
- Work with patients and their carers in order to support shared care planning and self-management.
- Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
- Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.
- Implement and participate in vaccination and immunisation programmes for both adults and children.
Communication
- Utilise and demonstrate sensitive communication styles, to ensure patients are fully informed and consent to treatment.
- Attend and contribute to nursing team, practice and primary care team meetings.
Delivering a quality service
- Recognise and work within own competence and professional code of conduct as regulated by the NMC.
- Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures.

Qualifications
- 1st level Registered Nurse with current registration with the UK Nursing Midwifery Council (NMC).
- Evidence of Continuous Professional development (CPD).
- Willingness to undertake prescribing qualification.
- Masters degree in a health related subject, or already working towards ANP Masters.
- Chronic Disease management.
Experience
- Experienced for 2yrs+ at ANP /specialist nurse level in an:
- Primary Care or another related area.
- Experience of working autonomously.
- Experience of working within a multi-disciplinary team.
- Evidence of team leadership and supervision of junior staff.
- Experienced in leading multidisciplinary teams.
- Experience of performance management of clinical staff members.
