Description And Responsibilities

Open Sky’s Behavioral Health Justice Involved program is a new model of care designed to assist MassHealth Members who are Justice- Involved--- either as current inmates or detainees, or on Justice Services such as probation or parole. The goal of the program is to support individuals in navigating and successfully engaging with health care services and behavioral health services. The program is designed to improve health outcomes and decrease fatal overdoses by providing connections to health care and community services.

The Justice Involved Navigator will provide In-Reach and Re-entry support, assessment, education, support and safety planning for individuals enrolled in the Behavioral Health Supports Justice Involved Program. They serve as a point of contact for integrated care coordination and care transitions in collaboration with individuals enrolled, justice agencies, medical and behavioral health providers, pharmacies, various social service agencies and other agencies and supports as indicated.

Other Key Responsibilities

  • Schedule and conduct outreach via in-reach visits within prescribed timeframes and engage eligible individuals by providing information about the BH-JI program.
  • Collaborate with enrollee’s justice entity, behavioral health, medical providers, and other collaterals to gather clinical information (including all existing medical, behavioral health and treatment plans) to complete a Bio-Psycho-Social Assessment and develop an Individualized Support Plan (ISP).
  • Coordinate access to services and fulfillment of integrated health needs including behavioral health and medical appointments as directed by the support plan.
  • Provide community supports by assisting the enrollee in navigating the network of community-based services and information.
Qualifications

  • High School Diploma, GED or equivalent, required.
  • Valid Driver's License, acceptable driving record and reliable transportation, required.
  • Experience with accessing local resources and navigating health systems, required.
  • Applicable experience in community-based behavioral health supports, substance use disorder services and community re-entry programs, required.
