Junior Clinical Fellow in Emergency Medicine (UECC)

Junior Clinical Fellow in Emergency Medicine 12 month post starting 6th August 2025

Rotherham District General Hospital

We are delighted to present this opportunity to join our growing emergency department in South Yorkshire seeing over 100,000 patients per year. We are a friendly district general hospital and Trauma unit caring for both adults and children. These posts offer an opportunity to gain first experience in emergency medicine or consolidate on that gained previously. The rota is self-rostered, 1 in 3 weekends, optional nights, and with 20% of paid time available to develop specialised interests to help build the career that you want. Options for that time include with our highly rated medical education team, ultrasound, Quality Improvement and as part of our work towards Green ED accreditation. For those unsure whether EM or GP is the future, your time can also be spent working with our friendly OOH GP team. We will also consider bespoke support for other interests or projects which develop your career.

Rotherham offers the best of three worlds. The town is surrounded by beautiful countryside and offers affordable housing in both urban and rural areas. It is also a 20-minute drive or 30-40 minute cycle from the bright city lights of Sheffield (9 miles from the hospital to the city centre) where you will find many of your potential colleagues.

Main duties of the job

We have 17 enthusiastic consultants keen to provide shop floor teaching and long-term Educational Supervision as you continue to develop. Our team of clinicians includes an F2, GPVTS, ACCS CT1s, EM HSTs, CESR Doctors and a fantastic cohort of ENPs and ACPs. We also have a Consultant dedicated to medical Education who provides bookable time for supervision and guidance on and off the shop floor to really take your skills to the next level.

Job responsibilities

The Department

At present the Urgent and Emergency Care Centre (UECC) at The Rotherham NHS Foundation Trust provides a 24-hour Emergency Service to a population in excess of 255,000. The centre co-locates the Primary Care and Urgent and Emergency Care Department. The total annual attendances are approximately 127,000 a year.

Large 6 bedded resuscitation room

A 4 bedded Rapid Initial Assessment area in Majors (RAT)

20 Adult Majors cubicles in two separate areas

Streaming and Triage Rooms along with rooms to carry out investigations from triage.

Minor area with assessment & procedure cubicles, eye/ ENT room, 10 large rooms housing Primary Care, Minor Injuries and rooms to see ambulatory patients for Emergency Medicine.

Self-contained Paediatric area with Paediatric assessment area, treatment room and five individual cubicles.

The Post

These posts have been created to give applicants the opportunity to gain or consolidate experience of emergency medicine. They will be contracted 40 hours weekly working in our friendly district General ED/trauma unit caring for both adults and children.

20% of paid time will be available to develop and pursue special interests. We are an expanding department with a strong mix of experienced and new Consultants and have good opportunities to pursue special interest in fields including but not limited to Medical Education and SIM, Ultrasound, QI and green ED. Appropriate individual bespoke options will also be considered.

The appointment is to commence in August 2025 for a fixed term period of 12 months. These posts are not recognised as training posts. The duties of this post are equivalent to an F2/ST1/ST2 Doctor.

Special interest options

Quality improvement

This is a key part of our departmental ethos and in the past 12 months trainees have successfully completed projects including: the use of age adjusted D-dimer in VTE, single shot Erector Spinae plane and SA blocks for rib fractures and the SNAP protocol hugely benefiting our patients and the trainees themselves. There are always projects ongoing that can be joined or lead by a successful applicant, but we also welcome new proposals.

We have three state of the art machines in the department. There are opportunities to learn or develop skills or projects under our ultrasound lead Dr A Madhok.

Medical Education and Simulation

We have an excellent reputation regionally regarding medical education both as a hospital and as a department. As a Medical Education Clinical Fellow you will get the chance to develop your educational skills, primarily through your expected participation in a number of areas of undergraduate training within the department. This will include showing leadership around the bedside experience of medical students in the department (January - September), facilitating their case based discussion tutorials (mostly Mondays), and assisting with their simulation sessions (most Thursdays). You will also have the opportunity to develop educational projects of your own, with this group, with the wider group of Sheffield medical students who are trained at Rotherham Hospital, and with postgraduate doctors in their early years. Support, training and feedback will be provided across all of these activities, if required, and can be tailored to help meet any requirements for educational qualifications being undertaken.

Green ED

The Climate crisis is a Health Crisis and as clinicians we are dedicated to reduce the impact of our work upon the planet and our patients. There are multiple ongoing projects aiming to achieve this and if desired clinical fellows would have many opportunities to help reduce our impact as we continue steps to Green ED accreditation. Previous projects including the adoption of Penthrox and scaling back Entonox to paediatric areas only. You could help drive the next project.

Urgent / Primary Care

For those interested in primary and urgent care we are a co-located Urgent and Emergency Care Centre. We work closely with the Urgent Care General Practitioners and Community Out of Hours team who are based in house. Applicants would be free to use their 25% to work with these colleagues on the Primary Care stream, on home visits or projects within this space. This is a great opportunity for those uncertain whether primary care or Emergency Medicine is the best fit for them, or those interested in Primary Care who want more exposure and experience in Urgent care.

Bespoke

If there is another specialty within the Trust you want more time or the opportunities to work with or shadow we will support this where possible. Additionally if you have other suitable projects that will help develop your skills and career as a clinician that would benefit from 20% of your clinical time this will also be considered.

CLINICAL DUTIES

Responsibilities:

The duties of this post are equivalent to ST1/ST2.

The successful candidate will take part in the Junior Doctor rota with 80% of their time being for clinical duties.

The appointed clinical fellow will be expected to assess and manage all categories of patients presenting to the Emergency Department, under the supervision of the Emergency Department Consultants.

The post holders will be responsible to the Consultants in Emergency Medicine, in the management of all patients presenting to the department.

Additional responsibilities will be in line with the chosen fellowship interest(s) of the successful candidates.

Key Tasks

To work as part of the multi-disciplinary team in order to support and help to provide a comprehensive Emergency Medicine Service within the Trust.

Maintenance of the highest clinical standards in the management of patients.

Assisting with teaching, training and assessment of junior staff and medical students, nursing staff and other members of the multidisciplinary team.

Main Responsibilities

Direct Clinical Care

Facilitate patient assessment, treatment and admission within the Emergency Department.

Liaison with other specialties and agencies including Primary Care within this process.

Requesting diagnostic tests (such as X-Ray and laboratory investigations) and taking biological samples, manually and through the Trust's computerised systems.

Act in accordance with the policies of the Emergency Department and the Trust and seek advice and assistance from the Consultants when necessary.

Ensure accurate and complete documentation is undertaken for all patient records in line with Trust protocols. This includes coding of clinical information.

Undertake consultations with relatives concerning patients when appropriate, maintaining confidentiality at all times.

When necessary accompany patients being transferred from the Emergency Department to other areas e.g. radiology department for CT or Intensive Care.

Communicate effectively with other healthcare professionals in and out of the Trust (e.g. specialty teams, GPs) about patient care when appropriate.

Study and Training

Each doctor has a nominated Consultant as a Clinical Supervisor who will regularly meet with them.

Hours of Work

The successful candidate will work a full shift pattern 40 hours per week, with dedicated time for pursuing their ED clinical fellow specialism.

The Tier 2 rota is self-rostered, under the supervision of Dr Matt Stafford (Emergency Department Rota Lead).

The rota is a 12-week annualised 1 in 3 weekends, no nights, latest shift finish 1am.

Break Entitlement

The rota is as a Full Shift Rota and as such Specialty Trainees can expect to be working for the whole duty period, except for natural breaks. The definition of a natural break is at least a 30-minute continuous break after approximately four hours' continuous duty. If, for any reason, you are unable to take a natural break during any shift you are required to complete an Exception Report Form outlining the reasons why you were unable to take a break. This will facilitate the department reviewing practice, as required, to ensure you continue to work in a safe environment.

Qualifications
  • Full registration and a licence to practice with the GMC (please confirm your GMC registration number).
  • MBBS or equivalent.
  • Eligibility to work in the UK.
  • ALS Provider.
  • Instructor candidate status for ALS.
  • APLS/EPALS.
  • ATLS or other trauma courses.
Research and Audit
  • Demonstrates understanding of the basic principles of audit, research, Quality Improvement and evidence-based practice, with potential to contribute to projects.
  • Active involvement in a clinical governance Programme.
  • Evidence of a portfolio of audit/quality improvement projects, including where the audit loop has been closed and where is evidence of learning of the principles of change management.
  • Evidence of understanding the practical workings of clinical governance e.g. handling a complaint, incident reporting.
  • Evidence of publication / presentation at conferences.
Education and Teaching
  • The ability and willingness to teach at all levels and to all groups of staff.
  • Experience of teaching junior staff/medical students.
