Integrated Care Pathway Project Coordinator (1.0 FTE ongoing contract)

The Middlesex London Ontario Health Team (MLOHT) is seeking a dynamic and motivated individual to join our team as an Integrated Care Pathway Project Coordinator. In this role, you will support team members, coordinate the work, and liaise between partners as we implement and sustain Integrated Care Pathways, including a focus on implementing the HealthPathways online platform for sharing, scaling, and sustaining integrated care pathways.

Reporting to: Middlesex London Ontario Health Team Lead

Partnering with: Burlington OHT, Greater Hamilton Health Network OHT, Streamliners

General Description:

The Integrated Care Pathway Project Coordinator will play a pivotal role in ensuring primary care and other partners contribute to and have access to integrated care pathways across Middlesex London. As a member of the Middlesex London OHT Operations Team, the Coordinator will work closely with a dedicated team of local healthcare providers and organizations to localize, develop, and continually improve and share integrated care pathways. In the HealthPathways work, the purpose of this role is to liaise between the local MLOHT team and the HealthPathways technical writing team, support the local team, and coordinate the development and maintenance of HealthPathways content and data.

Key Responsibilities

  • Coordinate a range of communications to support local team, partnering OHTs, and the HealthPathways team integrated care pathway processes
  • Use the HealthPathways System to manage and contribute to feedback on a daily basis and maintain pathway records and contact details
  • Act as the key point of contact for any communication between the local team and the HealthPathways team, including planning/facilitating/attending meetings to plan, monitor, and prioritize work/issues
  • Efficiently manage workflow processes and coordinate scheduled reviews of pathways.
  • Work with local partners to identify, gather, and share health system news for the region’s HealthPathways website home page
  • Enlist and coordinate clinical reference groups to engage in feedback (as relevant) and scheduled reviews for integrated care pathways
  • Gather, manage, update, and archive data in the HealthPathways Directory
  • Work with the HealthPathways team to ensure information displays correctly
  • Organize HealthPathways user engagement activities across the local health system
  • Produce and share integrated care pathway reports for key stakeholders
  • Be a champion for “health system change” and how integrated care pathways and the HealthPathways platform can support, including delivering presentations
  • Ensure that care pathway localization includes a focus on equity, culturally safe care, wholistic care, and supports for people impacted by the social determinants of health
  • Establish and maintain relationships with partnering OHTs, Streamliners, and other key stakeholders
  • Support other related MLOHT strategic priorities, as needed

Qualifications and Experience

  • Bachelor's degree in Health Administration, Health Management, Public Policy/Administration, Human Resources, Project Management, or a related field.
  • Proven experience in health system improvement work, including stakeholder management; project management; process mapping and working with multiple sectors; working with primary care and specialty care preferred
  • Experience project managing multiple projects within a portfolio/projects with multiple work streams
  • Knowledge of the healthcare system in Ontario, strong understanding and connection with MLOHT work preferred
  • Excellent interpersonal and collaboration skills and the ability to build and maintain trusting relationships with patient/clients, caregivers, and providers; ability to leverage existing strong cross-sectoral relationships with MLOHT partners preferred
  • Exceptional organizational and time-management abilities to handle multiple tasks simultaneously
  • Excellent active listening skills, with a relentless commitment to seeking to understand
  • Able to plan and schedule activities in a complex environment
  • Ability and desire to work independently and as part of a collaborative team
  • Ability to adapt and resilience to changing circumstances
  • Awareness of how to market to/engage primary care
  • Overall knowledge of the health and social services available in Middlesex London preferred

Salary: Dependent on experience

This is a hybrid role, combining both in-office and remote work, with occasional travel required throughout the region as needed.

Secondment arrangements can be considered.
