Integrated Care Pathway Manager (1.0 FTE, ongoing contract)

The Middlesex London Ontario Health Team (MLOHT) is seeking a dynamic and motivated individual to join our team as an Integrated Care Pathway Manager. In this role, you will lead the local implementation and sustainability of Integrated Care Pathways, including a focus on implementing the HealthPathways online platform for sharing, scaling, and sustaining integrated care pathways.
General Description/Purpose:
The Integrated Care Pathway Manager will play a pivotal role in ensuring primary care and other partners contribute to and have access to integrated care pathways across Middlesex London. As a member of the Middlesex London OHT Operations Team, the Manager will work closely with a dedicated team of local healthcare providers and organizations to localize, develop, and continually improve and share integrated care pathways. In the HealthPathways work, the purpose of this role is to oversee the day-to-day management of the local HealthPathways team and ensure successful implementation of this online platform, including localization, engagement, and coordination of health system improvements related to the pathways.
Key Responsibilities:
- Provides strategic oversight and ensures that planning and implementation results in embedding HealthPathways across the local health and social care system.
- Provides operational leadership to the MLOHT Integrated Care Pathways team.
- Oversees our obligations under the HealthPathways Service Agreement and standards and manages the budget.
- Ensures efficient and effective selection and localization of integrated care pathways.
- Develops integrated care pathway implementation targets, scope, resourcing, and scheduling.
- Develops and maintains a detailed plan to manage and track progress.
- Manages risks, resolves issues and conflicts, and escalates to management, as required.
- Drives performance metrics on pathway localization and user engagement.
- Ensures that care pathway localization includes a focus on equity, culturally safe care, wholistic care, and supports for people impacted by the social determinants of health.
- Reports progress to the Executive Sponsor and other key/designated stakeholders.
- Establishes and maintains relationships with local clinicians and subject matter experts, partnering OHTs, Streamliners, and other key stakeholders.
- Liaises with local, regional, and provincial health system partners to support implementation and scale of HealthPathways.
- Identifies and builds relationships with local clinical champions and subject matter experts.
- Champions Integrated Care Pathway work and engages system partners in design and action.
- Identifies alignment and integration opportunities with other MLOHT strategic projects and works across collaborative teams toward a broader integrated care strategy.
Qualifications and Experience:
- Bachelor's degree in Health Management, Public Policy/Administration, Human Resources, Project Management, Health/Business Administration, or a related field.
- Proven experience in health system improvement work, including process mapping and working with multiple sectors; working with primary care and specialty care preferred.
- At least 5 years of project management experience in healthcare.
- Knowledge of the healthcare system in Ontario, strong understanding and connection with MLOHT work preferred.
- Excellent interpersonal and collaboration skills and the ability to build and maintain trusting relationships with patients/clients, caregivers, and providers; ability to leverage existing strong cross-sectoral relationships with MLOHT partners preferred.
- Exceptional organizational and time-management abilities to handle multiple tasks simultaneously.
- Excellent active listening skills, with a relentless commitment to seeking to understand.
- Experience managing, motivating, and influencing teams.
- Outstanding verbal and written skills, emphasizing professionalism and thoroughness; strong public speaking/presentation skills.
- Ability and desire to work independently and as part of a collaborative team.
- Ability to adapt and resilience to changing circumstances.
- Strong analytical skills and ability to drive results.
- Strong data visualization and reporting skills.
- Strong problem-solving skills.
- Co-design/Experience Based Design experience an asset.
- Overall knowledge of the health and social services available in Middlesex London preferred.
Salary: Dependent on experience.
This is a hybrid role, combining both in-office and remote work, with occasional travel required throughout the region as needed.
Secondment arrangements can be considered.
