The Urgent Community Response Team (UCR) provides a rapid assessment, diagnostic and treatment service to reduce unscheduled admissions for people with complex health needs, exacerbation of long-term conditions and frailty. The Hospital @ Home (H@H) model is for longer length of stay and treatment under the UCR/H@H service. Patients referred to the UCR service are seen within 2 hours, and those applicable transferred to the Frailty Hospital at Home. The service aims to provide hospital level care at home and includes urgent case management responding to crisis, recovery support, self-management, and adapt a collaborative approach with patients. The Mid Hampshire UCR team operates from a hub in Colden Common, located near Winchester. UCR Mid Hampshire delivers a joined-up approach across Winchester and Andover locality, providing excellence in care and engage with all GP practices, South Central Ambulance Service, community district nursing teams, local acute trusts and wider stakeholders in the locality. The service runs 0800-2000 7 days a week, 365 days a year.

Main duties of the job

The post holder will work as an active member of the Urgent Community Response Team/Frailty Hospital at Home to provide a high quality, efficient and effective service to patients. The post holder will provide administrative/clinical support for the running of the hospital at home model and the clinical team maintaining the effectiveness in a fast-paced environment. The post holder will be responsible for the accurate management of the Hospital at Home for both the team and the service users. The post holder will be proficient, knowledgeable, and competent. They will use problem solving skills to identify issues and resolve them or seek help to ensure the smooth running of the service. The post holder will continue to develop their role through the acquisition of further skills, knowledge, and competences, to be determined by the needs of patients, the service, and the team. The post holder will have experience of working in primary care/community, ideally in health, social care or other support roles including direct contact with patients, their families, and carers in a related role. Applicants will need excellent administration, interpersonal and communication skills and be organised, patient and empathetic. The post is full time (37.5 hours per week) and allows for flexibility through working a combination of home and office workplace options (Hybrid). The post holder will be expected to work flexibly 8 hrs a day across a 7 day period between the hours of 8am-6pm (With 30mins unpaid lunch).

Person Specification

Qualifications

  • Good secondary education grades, to include English Language grade C or equivalent
  • English and Maths A-levels or equivalent. NVQ 3 Business Administration or equivalent evidence in practice.
  • Able to demonstrate a good command of the English Language both written and verbal.
  • Computer Literate in both data entry and communicating via IT - emails and reports MS Teams, Outlook, Excel, Word, Power Point
  • Able to use graphic design software in accordance with Trust publications guidelines ie CANVA. Previous experience in working in an NHS setting community Team/ administration team/GP. Experience of using RiO and other NHS Systems including Tableau, Ulysses, EMIS etc. Access database experience.
