HOSPICE PHYSICIAN
BAYADA Home Health Care Hospice is seeking to fill the position of part-time Hospice Physician for our VT Locations. These offices provide best-in-class care to our hospice clients throughout the state of VT and 1 location in NH. This is a Part-Time position (about 8-10 hours weekly). Must have a valid VT/NH license.

PRIMARY RESPONSIBILITIES:
Perform those services appropriate to the position of Associate Medical Director of a hospice, which include, but are not limited to, the following administrative services: a) Participate in Interdisciplinary Group (IDG) to coordinate all aspects of hospice care, including participation in initial and ongoing IDG comprehensive assessments. b) Certify and recertify client eligibility for hospice care. c) Ensure that the hospice Plans of Care (POC) are medically appropriate and consistent with federal and state laws and regulations in accordance with applicable Medicare hospice regulations.
As an accredited, regulated, certified, and licensed home health care provider, BAYADA complies with all state/local mandates.
