We are searching for a talented RN - Supportive Care
*UP TO $12,000 NEW HIRE SIGN ON BONUS!*
Hours: 40hrs
Shift: 10a-6:30, 11a-7:30 or 12-8:30pm with weekend and holiday rotation; Every 4th weekend, 2 holidays per year
Location: Southcoast Business Center - Fairhaven, MA

Under the supervision of the Director, Supportive Care Services or designee, and within agency and department policies and procedures, assume responsibility for the primary care of assigned Hospice and Palliative Care patients and families. Assess, develop, implement and evaluate an individualized plan of care to maximize the comfort and health of the Hospice and Palliative Care patient and family. Responsible for identifying patient/family needs and for providing supportive care in accordance with the attending Physician’s orders, Plan of Care and Hospice and Home Care policies and procedures.
USD $36.90 - USD $52.64 /Hr.
Every 4th weekend, 2 holidays per year
