Home Therapies Clinical Nurse Consultant

Alfred Health is a leader in health care delivery, improvement, research, and education. We are the main provider of health services to people living in the inner southeast suburbs of Melbourne, from ambulatory to inpatient and home and community-based services.

Permanent Full time (80hrs per f / n) + ADO's

Clinical Nurse Consultant C - ZA7 / ZA8 -CAPR 4.1 / CAPR 4.2

Salary : $125,288.80 - $128,268.40 p / a + 11.5% Superannuation

About the role

We are seeking an enthusiastic and experienced Home Therapies CNC to utilise their expert clinical knowledge to provide advanced patient assessment, training, and management for patients who dialyse independently in their homes. The Home Therapies CNC will work collaboratively with nursing and medical staff, particularly the Home Therapies Medical Lead, as well as the wider multi-disciplinary team to optimise patient care in a safe, effective, and timely manner, including management of acute and urgent start dialysis.

A key priority of this role is to further optimise home therapy options for suitable patients across both PD and HHD. The CNC will have a leadership role within Home Therapies, focusing on clinical services, education, workforce, quality and safety, and research. The Home Therapies CNC will ensure the Home Therapies team facilitate all aspects of assessment, education, and ongoing care of patients on home therapies in a timely manner, ensuring quality improvement and equity in access to services.

About you

  • Current Registration as a Registered Nurse with Nursing and Midwifery Board of Australia via AHPRA.
  • Minimum of 5 years experience in Renal nursing.
  • Post graduate qualification in Renal Care preferred / working towards Masters qualification.
  • Ability to work collaboratively with other services and departments across the hospital and community.
  • Ability and willingness to develop others.
  • Commitment to quality and safety activities / research process and application to practice.
