Healthcare Assistant - NHS Health Checks

We are looking to recruit a highly motivated and innovative Health Care Assistant with experience of working in a clinical setting to work within the Rotherham area, delivering Health Checks into external workplaces, GP practices and screening the health of employees/patients across the borough of Rotherham. You will also be expected to support employee/patients by giving health advice face to face as well as signposting and cross-referring to other support services.

You will be expected to work within the team, but also independently in external workplaces to carry out this role, following specific policies, guidelines and protocols. There will be lone working involved.

Your role will be delivered across various workplaces/GP practices in Rotherham; therefore, access to a suitable vehicle and the ability to travel to a range of venues and settings is essential.

We are looking for someone who has experience of delivering NHS Health Checks (albeit training can be given) and is able to evidence an understanding of Public Health initiatives and programmes.

Main duties of the job

1. Completion of point of care health check and questionnaire incorporating BMI, BP, GPPAQ, AUDIT-C, Cholesterol/HDL and CVD for employees/patients in their place of work or GP practice.
2. You will be making contact with employees face to face, so effective communication skills and professionalism are essential.
3. You will be signposting and cross-referring to other support services where applicable.
4. To deliver at point of care advice around improving cardiovascular health and providing easy to understand analysis of the outputs from the health check.
5. Requesting feedback from the employees.
6. Questionnaires and health check data will be completed online, full training on the software will be delivered; however, basic computer skills will be required. The output from this will often feed into SystmOne, where it does not, a personalised health report will be provided to the employee.

The organisation has been in place for approximately 6 years and employs a range of primary care network additional roles (ARRS) and staffing for primary care services in Rotherham.

Job responsibilities

The NHS Health Check Assessors role includes supporting and complying with processes, procedures and assisting in achieving the NHS Health Checks targets, which relate to other services such as weight management, the behaviour change and well-being coaches' goals. You will need to fulfil the Health Check Assessors criteria for internal and external performance management, including weekly reporting.

It is important that you make sure data is inputted rigorously and consistently to guarantee that accurate records are kept up to date in compliance with Connect Healthcare's procedures and information compliance.

Governance and data protection is essential in relation to patient information. It is essential that data is kept private and secure at all times. This is something you will need to consistently adhere to.

The role is ideally suited to people with strong communication skills and those who are really passionate about making a difference to people's lives on a daily basis. It also includes working with vulnerable adults and such the successful candidate will be required to apply for a DBS prior to starting the role.

Person Specification

Qualifications

• GCSE Maths and English to at least Grade C, or equivalent. Venepuncture certification. Knowledge on NHS Health Checks. Qualified in vital observations.
• Recognised Health Trainer qualifications e.g. NHS Health checks or equivalent, or Understanding Health Improvement or equivalent.

Experience

• Experience in providing health-promoting initiatives/services in the local community. Experience in working at events. Experience in working in community and GP settings. Knowledge in completing NHS Health checks. A good knowledge of the Rotherham area. You will need to be a people-person, an enthusiast for how healthy lifestyles can impact positively on people's health, someone who is fantastic at building rapport with an ability to persuade.

Knowledge requirements

• Good knowledge of challenges to achieving health improvements amongst vulnerable and deprived communities. Good knowledge of application of behavioural change theoretical principles, to facilitate, motivate and empower individuals / local communities to make substantive and sustainable behaviour change. Training can be considered for the right candidate. Knowledge and certificate on NHS Health checks.

Special Conditions

• Full UK Driving license with access to a car with business travel insurance for work. This role incorporates working evenings and weekends with various levels of travel across Rotherham, working in a variety of settings including primary care and community centres. This post involves working with young people/vulnerable adults and as such the post holder will be required to apply for a disclosure of criminal records at an enhanced level (DBS).
