We are looking to recruit a highly motivated and innovative Health Care Assistant with experience of working in a clinical setting to work within the Rotherham area, delivering Health Checks into external workplaces, GP practices and screening the health of employees/patients across the borough of Rotherham. You will also be expected to support employee/patients by giving health advice face to face as well as signposting and cross-referring to other support services.
You will be expected to work within the team, but also independently in external workplaces to carry out this role, following specific policies, guidelines and protocols. There will be lone working involved.
Your role will be delivered across various workplaces/GP practices in Rotherham; therefore, access to a suitable vehicle and the ability to travel to a range of venues and settings is essential.
We are looking for someone who has experience of delivering NHS Health Checks (albeit training can be given) and is able to evidence an understanding of Public Health initiatives and programmes.

The organisation has been in place for approximately 6 years and employs a range of primary care network additional roles (ARRS) and staffing for primary care services in Rotherham.
The NHS Health Check Assessors role includes supporting and complying with processes, procedures and assisting in achieving the NHS Health Checks targets, which relate to other services such as weight management, the behaviour change and well-being coaches' goals. You will need to fulfil the Health Check Assessors criteria for internal and external performance management, including weekly reporting.
It is important that you make sure data is inputted rigorously and consistently to guarantee that accurate records are kept up to date in compliance with Connect Healthcare's procedures and information compliance.

The role is ideally suited to people with strong communication skills and those who are really passionate about making a difference to people's lives on a daily basis. It also includes working with vulnerable adults and such the successful candidate will be required to apply for a DBS prior to starting the role.
