Job Summary

Care Plus Group (CPG) works in partnership with Navigo to deliver Neurodiversity Services for people with Learning Disabilities and Autistic people within North East Lincolnshire and across the Humber TCP. These services aim to meet the unique needs of these populations through the provision of various Assessment and Treatment, Intervention and Support Pathways using a Psychology-led, Multi-Disciplinary, needs-led, person-centred approach.

The wider Adult Neurodiversity Services includes an LD Psychology Service, Intensive Support Team, Community Learning Disability Team, Therapies Team, Carers Service, Forensic Outreach Liaison Service, Adult Autism Diagnostic and Support Service and an ADHD Diagnostic Service. All these services are provided via a person-centred, needs-led model.

Health and Wellbeing Co-ordinator

Community Learning Disability Team

37.5 hours per week (Mon to Fri) - Fixed Term for 12 Months

Salary Range 4 £24,645 - £27,202 per annum you will be appointed on a fixed salary point within the range.

We are seeking a highly skilled and motivated individual to join our Team to maintain and drive the development of high-quality services for people with a learning disability. This is a rare and exciting opportunity to become part of a motivated and passionate multi-disciplinary Community Learning Disability Service.

As a Health and Wellbeing Coordinator you will be responsible for supporting people with learning disabilities to get their health needs met. You will be responsible for liaising closely with other health professionals, developing health action plans, supporting people to attend health appointments, health screening and healthy activities. In addition, you will also support people with learning disabilities and their carers to understand good health and wellbeing, and you will also support mental capacity and best interest decision-making processes.

You must be well motivated, a self-starter and possess an NVQ 3 in health and social care or equivalent and have a minimum of 2 years experience of working with people with a learning disability.

Skills

Essential

  • Effective written and verbal communication and willingness to adapt communication to meet the needs of people with limited verbal communication and/or sensory needs.
  • Ability to prepare written reports.
  • Effective IT skills in the use of word, Excel, email.
  • Good organisational skills and ability to work to deadlines.
  • Ability to plan and prioritise own workload and work independently.
  • Knowledge of Valuing People, Mental Capacity Act, Community Care Act, 74 Deaths and Counting Report and other legislation pertaining to job role.
  • Ability to find out how to access activities to support healthy lifestyles.

Desirable

  • Knowledge of local NHS structures.
  • Group work and facilitation skills.

Qualifications

Essential

  • NVQ Level 3 Qualification, preferably in health or social care or equivalent, plus evidence of additional training through courses.
  • Relevant Undergraduate Degree.

Desirable

  • Professional Health or Social Care Qualification or degree.
  • C&G 730 Further Adult Teaching Certificate or equivalent.
  • Qualification in healthy activities i.e. trampoline coach.

Experience

Essential

  • Minimum of two years experience working in a health or social care setting.
  • Experience of working with people providing support and/or information designed to improve their quality of life.
  • Experience of planning, organising and delivering events or activities for others' benefit.
  • Experience of working within a team of people who share a common goal.

Desirable

  • Experience of writing Care/Support Plans or Health Action Plans.
  • Experience of working with people with a range of complex health needs i.e. epilepsy, sensory difficulties.
  • Experience of designing and facilitating training sessions.

Other

Essential

  • Able to transport self to locations within North East Lincolnshire.
  • Willingness to undertake further training.
  • Work flexibly to meet the needs of service users and carers.
