Job responsibilities

2. Conduct health checks and routine measurements as directed by healthcare professionals.
3. Collaborate with the proactive care team to identify and implement preventive care measures.
4. Support patients in managing their health conditions through education and guidance.
5. Maintain accurate and up-to-date patient records, ensuring confidentiality at all times.

Person Specification

Experience

• Relevant qualification as a Health Care Assistant working in general practice care certificate.
