The new services have been developed to streamline the coordination of care for patients, address the evolving palliative and end of life care needs of our local population and maximise the flexibility and responsiveness we can offer.

We are looking for an experienced, enthusiastic, and highly motivated individual to take up the post of the Head of Clinical Hub in our Clinical Hub Services. This post is suited to a senior nurse with significant experience of caring for patients with life limiting illnesses who possesses advanced knowledge and skills in clinical assessment, symptom management and communication.

Main duties of the job

Responsibilities of the Head of Clinical Hub

  • Provide visible and effective leadership and management to all directly and indirectly managed staff which will achieve purpose and Trinity's vision and values.
  • Develop and maintain an organisational culture which is inclusive and empowering, promotes trust, respect and responsibility and delivers positive and sustainable change.
  • Develop and maintain organisational systems and processes which support the delivery of high-quality integrated patient care, team working and communication.
  • Establish and maintain relationships with external stakeholders and represent the organisation in a professional and collaborative manner.
  • Responsible for overall staff management within both services; ensure that all staff are recruited, inducted, managed and supported in line with Trinity's policies and procedures.
  • Actively contribute to the Patient Services management team and hospice Senior Leadership Team, taking individual and corporate responsibility for effectiveness and development.

Job responsibilities

Overall Purpose of the role

The provision of strategic and operational leadership and management of the Admission and Referrals services and Wandsworth End of Life Care Coordination services (WEoLCCS). The post holder will lead on ensuring these services meet the needs of the local population, are accessible and have clear patient pathways and processes.

Person Specification

Qualifications

  • Professional qualification and associated membership of a professional body. Registered with the NMC.

Experience

  • Significant experience of nursing and service management within a health care sector setting including recruitment and retention.
  • Experience of providing nursing leadership at a senior level and knowledge of relevant professional nursing issues.
  • Significant experience of providing direct patient services.
  • Experience of developing and maintaining effective inter-disciplinary team working structures and systems.
  • Experience of managing challenging scenarios.

£61,927 to £68,676 a year includes high cost area supplements (HCAS).
