Job description

St Pauls Primary Care Network presents an exciting opportunity for an experienced Health Care Assistant to work across the four practices that form the Network, all housed in St Pauls Medical Centre in the heart of Cheltenham.

We are offering a part time position of 24 hours a week.

Main duties of the job
  • Provide direct care and personalised support to patients needing Long Term Condition and Health Checks particularly: Learning Disability, Serious Mental Illness, Diabetes, Hypertension, Asthma, Dementia, AF.
  • Provide a range of clinical diagnostic skills in order to provide annual health checks, recording onto the patients' record and providing onward care and information.

When and where necessary, provide care to patients in routine clinics.

St Pauls Primary Care Network is made up of four practices that form the Network, all housed in St Pauls Medical Centre in the heart of Cheltenham (Weston House, St Catherines Surgery, St Georges Surgery, and the Royal Well Surgery). We have a fantastic team of Allied health professionals including paramedics, ANP's, social prescribers, health care assistants, FCP, pharmacists, and pharmacy technicians.

Job responsibilities

Long Term Conditions & Health Checks

  • Provide a range of clinical diagnostic skills in order to provide annual health checks, recording onto the patients' record and providing onward care and information.
  • Provide a full health check including diagnostics and phlebotomy.
  • Provide patients with immunisations both seasonal and long term.
  • Attend the MDTs, recording and following up actions within defined timescales agreed during the meeting.
  • Work with members of the primary care teams, to develop and implement proactive and reactive care and ensure data collection systems are completed accurately and in a timely manner.
  • Raise awareness within the PCN to shared decision making and decision support tools.

There may be, on occasion, a requirement to carry out other tasks; this will depend on factors such as workload and staffing levels:

Experience
  • Experience of working with the general public & patients
  • Experience of working in a healthcare setting or in the leisure industry / public / voluntary sector
  • Experience of working with vulnerable and elderly people in their own homes
  • Experience of working in primary care
Qualifications
  • Educated to GCSE level or equivalent
  • NVQ Level 2 in Health and Social Care or other relevant professional academic qualification (Health Care Assistant)
  • Qualified in Phlebotomy
