Job Overview:
We are seeking an experienced and compassionate Practice Nurse to join our dynamic primary care team at Mill Hill Surgery. As a key member of our multidisciplinary team, you will provide high-quality nursing care to our patients, promoting health and wellbeing in the community. The ideal candidate will have a broad range of clinical skills, excellent communication abilities, and a strong commitment to patient-centered care.
Our practice, Mill Hill Surgery in Ealing / Acton, is composed of six (6) GP partners, along with a Nursing team including HCA support, a newly appointed GPA role, and a comprehensive admin support team providing primary care services to a registered population of approximately 7000 patients across the borough of Ealing.

Key Responsibilities:
As a Practice Nurse, your duties will include, but are not limited to:
- General Nursing Duties:
- Provide general nursing care for patients in the practice, ensuring a high standard of clinical practice.
- Assess, plan, implement, and evaluate patient care in line with clinical guidelines and best practices.
- Carry out basic wound care, dressings, and management of minor injuries.
- Provide health education and advice, promoting self-care where appropriate.
- Chronic Disease Management:
- Provide care and support to patients with long-term conditions such as asthma, COPD, diabetes, hypertension, and cardiovascular diseases.
- Conduct routine assessments, monitoring, and management plans for patients with chronic illnesses.
- Offer lifestyle advice and coordinate care in collaboration with other healthcare professionals.
- Immunisations and Vaccinations:
- Administer childhood and adult immunisations as part of the vaccination schedule.
- Provide travel vaccinations and advice on travel health.
- Participate in seasonal vaccination campaigns (e.g., flu clinics).
- Health Screening and Assessments:
- Carry out health checks for patients, including blood pressure monitoring, phlebotomy, and weight management.
- Conduct cervical smears and sexual health screening.
- Minor Illness and Injury Management:
- Offer wound care and first aid, including assessment of minor injuries and referral when appropriate.
- Medication Management:
- Ensure accurate documentation.
- Monitor patients on specific medications, such as anticoagulants or hormone injections, and arrange for regular reviews.
- Patient Education:
- Educate patients on disease prevention, healthy living, and self-management of conditions.
- Provide emotional support and guidance for patients with chronic illness or other complex health needs.
- Administrative Duties:
- Maintain accurate, up-to-date patient records, in line with legal and confidentiality requirements.
- Coordinate patient appointments, ensure efficient patient flow, and contribute to the smooth running of the practice.
- Participate in audit and quality improvement initiatives to enhance patient care and practice performance.
- Multi-disciplinary Team Collaboration:
- Work closely with GPs, healthcare assistants, and other members of the healthcare team to deliver integrated patient care.
- Participate in clinical meetings and support the implementation of practice-wide policies and procedures.
- Provide support and mentorship to other staff members as needed.
Qualifications & Experience:
- Registered Nurse with valid NMC registration.
- Minimum of 1 years of clinical experience in a general practice, primary care.
- Experience in chronic disease management, immunisations, and general nursing duties.
- Qualification in cytology and immunisations.
- Additional qualifications in areas such as diabetes management, asthma care, or travel health are desirable.

- Excellent communication skills, with the ability to explain complex information in a clear, empathetic manner.
- Ability to manage a busy workload, prioritise tasks, and work efficiently under pressure.
- Knowledge of current clinical guidelines, health promotion strategies, and public health initiatives.
- Ability to work collaboratively as part of a multi-disciplinary team.
- A proactive approach to continuing professional development (CPD) and keeping up-to-date with evidence based practice.
