Frailty Practitioner - Urgent Community Response
Hampshire and Isle of Wight Healthcare NHS Foundation Trust

Frailty Practitioner (Nurse or Paramedic) -- Urgent Community Response (UCR) and Hospital at Home Frailty Virtual Ward Team (H@HVW)

We're looking for a Registered Nurse or Paramedic to join our Urgent Community Response (UCR) team, and Hospital at Home Frailty Virtual Ward Team (H@HVW) delivering rapid, high-quality care to patients in their own homes and preventing hospital admissions where safe to do so.

As a Frailty Practitioner, you'll work closely with Advanced Clinical Practitioners (ACPs), trainee ACPs, Consultant Practitioners, Physiotherapists, Occupational Therapists, and Health Care Support Workers to provide timely treatment and care planning following initial assessments by senior team members.

Main duties of the job

What You'll Be Doing:

  1. Supporting admission avoidance by delivering treatment and care planning after an initial assessment by senior team members.
  2. Collaborating with the Multidisciplinary Team (MDT) to provide person-centred care.
  3. Delivering interventions at home to include but not limited to holistic frailty reviews placing the person at the centre of care, re-assessing and monitoring treatment plans, point of care testing, IV fluids, IV antibiotics, ECG's.
  4. Delegating tasks and supervising junior staff.
  5. Working on the Falls Car, alongside SCAS, to assist uninjured fallers and ensure safe recovery at home with a full holistic and functional review.

Our service is delivered over 7 days, 8am-8pm. We offer flexible working, but the ideal candidate will be able to work flexibly within these hours including working 1 weekend in 4.

Person Specification
Qualifications
  • Professional UK registration e.g. NMC / HCPC
  • Evidence of Continued Professional Development (CPD) clearly recorded for professional profile
  • Able to demonstrate a good command of the English Language both written and verbal
  • Computer Literate in both data entry and communicating via IT - emails and reports
Experience
  • Experience working with frail patients or those with exacerbation of long-term conditions.
Additional Criteria
  • Full driving license and access to a car
  • Ability to meet the travel requirements of the post which will include travel to other areas of the trust for training and home visits to patients place of residence

Salary: £37,338 to £44,962 a year based on full time hours.
