Job description

Service Care Solutions are recruiting for an Enhanced Frailty Nurse Practitioner to support elderly people aged 65 years and older, within the community.

Enhanced Frailty Nurse Practitioner
Pay: £35 LTD p.h
Contract: Locum for 8 weeks
Start: ASAP
Location: Harrow, Greater London

Essential Qualifications:

1. BSc Nursing or Emergency Care Practitioner Qualification
2. Non-Medical Prescribing Qualification
3. Wound Care, including compression bandaging
4. Phlebotomy
5. Community working

Duties:

1. To reduce avoidable hospital admissions or re-admission or A&E attendances
2. Provide clinical support to patients, supported by the wider Clinical team, to work autonomously holding their own caseload.
3. Support the Enhanced Frailty Team in the delivery of Complex Care.
4. Attend all mandatory meetings such as Touch Point meetings and MDTs.
5. To maintain excellent working relationships with all those involved in the provision and development of services for those with long-term conditions.
