Field Care Manager - North West Region

The organization is seeking a passionate and dedicated Field Care Manager to oversee the delivery of a high standard of home care services in the North West region, including North Wales, Liverpool, Manchester, Stoke, and surrounding areas.

Main duties of the job

As a Field Care Manager, you will be responsible for conducting client assessments, developing and reviewing personalized care plans, liaising with the operational team, ensuring compliance with CQC standards, building relationships with clients and their families, and collaborating with healthcare professionals to provide holistic care. The role involves regular and frequent travel to clients' homes and may include some evenings and weekends.

Job responsibilities

Job Title: Field Care Manager
Location: North West
Salary: £27,000 Per Annum
Reports to: Senior Clinical Lead for Region
Working Hours: 40 hours per week (with some flexibility over weekends/evenings depending on client needs)

Key Responsibilities:

• Care Planning & Assessment: Develop and review personalised care plans.
• Liaise with the operational team regarding performance issues and training needs.
• Compliance & Quality Assurance: Ensure services meet CQC standards, conduct audits, and maintain accurate records.
• Build relationships with clients and their families, address concerns, and update their care needs.
• Collaborate with healthcare professionals to ensure holistic care.

Requirements:

• NVQ Level 3 or 4 in Health & Social Care (or equivalent).
• Experience in a supervisory/management role within health and social care.
• A passion for delivering high-quality care, exceptional communication, and organisational skills.
• Full UK driving license and ability to travel using own car.
• This role requires travel around the North West including North Wales, Liverpool, Manchester, Stoke, and surrounding areas.

• Working hours – 9am-5:30 pm Monday to Friday with some flexibility over weekends/evenings.
