We are looking for an Elderly Care Coordinator to join our growing team at Our Health Partnership, working 20 hours per week for and across the Kingstanding, Erdington & Nechells Primary Care Network. The PCN consists of 7 practices covering the areas of B7, B44, B10 and B23.

Please note, our head office is in Cotteridge but you will be based at a location within the PCN.
Main duties of the job
A key part of the role of a care coordinator role is improving the continuity of care by acting as a point of contact for residents, families and professionals who are housebound. Ensuring that they receive the same level of care as our non housebound patients, putting in measures to support and enable them to still have access to services. Identification of people in need of review, or collation of information on people requiring an MDT review in addition to providing coordination, secretarial and administrative support to the MDTs.
A good understanding of General Practice, Primary Care Networks and elderly care is desirable but not essential as full training will be provided. This role would suit someone who is a forward-thinking, innovative, and enthusiastic individual who is self-motivated and keen to work collaboratively across the primary care network. You should have the ability to be organised, prioritise your workload effectively and exercise sound judgement when faced with conflicting pressures.

The Care Coordinator will be part of the Primary Care Network (PCN) Multi-Disciplinary Team (MDT) who are responsible for managing the care of people registered with practices within a particular PCN. This will involve coordinating the work of healthcare professionals and non-clinical staff including volunteers involved in the care of patients registered at GP practices within the wider PCN population.
The post holder will contribute to tackling inequalities in health and social care particularly regarding elderly individuals. An ethos of promotion of independence and partnership-working is integral to this post.
