Location: The Brook Surgery, Crowborough Hill, Jarvis Brook, Crowborough TN6 2ED
Reports to: ACP Clinical Lead.
Salary: £30 p/hr.
Employment Type: Full-Time/Permanent - Surgery Based (not remote)

Are you a passionate and experienced Clinical Prescribing Pharmacist looking for an opportunity to make a real impact in primary care? We are seeking a dedicated professional to join our dynamic multi-disciplinary team, focusing on medicines management and patient-centred care.

About the Role:

This role is designed to enhance patient care through expert clinical practice, prescribing, and leadership in medicines management. You will play a key part in optimising medication use, managing long-term conditions, supporting polypharmacy reviews, and ensuring safe, effective prescribing practices.

Key Responsibilities:
  1. Prescribing & Medicines Management – Lead safe and effective medication optimisation, including structured medication reviews.
  2. Collaborative Working – Work closely with GPs, nurses, and other healthcare professionals to deliver integrated patient care.
  3. Patient-Centered Care – Support patients in managing their conditions and improving health outcomes.
  4. Education & Training – Provide guidance to the wider clinical team on best prescribing practices.
  5. Quality Improvement – Drive initiatives to enhance medication safety and effectiveness.
What We’re Looking For:
  • Qualified Prescribing Pharmacist (or other registered health professional) with primary care experience.
  • Strong background in medicines optimisation and long-term condition management.
  • Ability to work independently and collaboratively in a fast-paced clinical environment.
  • Advanced physical assessment skills are desirable but not essential.
Clinical Practice:
  1. Conduct comprehensive patient assessments, including history-taking and physical examinations.
  2. Develop and evaluate personalised care plans, with a focus on medicines management.
  3. Prescribe and optimise medications in line with best clinical practices.
  4. Provide expert guidance on safe and effective medication use to patients and colleagues.
Medicines Management:
  1. Lead initiatives to enhance medication safety and reduce errors.
  2. Develop and implement medicines management policies.
  3. Conduct clinical audits and drive quality improvement projects.
  4. Collaborate with pharmacy, nursing, and medical teams to optimise prescribing.
Education & Leadership:
  1. Educate and mentor healthcare staff, fostering continuous learning.
  2. Contribute to the development of clinical guidelines and best practices.
  3. Represent the organisation at relevant forums on medicines management.
