Senior Community Support Worker – Milton Keynes

Location: Milton Keynes

Hours: Full-time, Part-time, Contract, Permanent

Your role will involve working in all areas of Milton Keynes & surrounding areas. Therefore, the ability to drive and a FULL DRIVING LICENSE is very desirable.

As a Senior Community Support Worker, you will provide guidance, motivation, support, and supervision to our dedicated and exceptional community support worker team. You will monitor adherence to policy and procedure and ensure that our values are always upheld. You will provide direct person-centered care to our valued and diverse group of service users.

Some of your care duties include:

  • Supporting individuals to access their community
  • Developing relationships and liaising with external health care professionals such as GP, social workers, or community nurses.
  • Acting as key worker to a small group of service users
  • Conducting service user reviews
  • Accompanying service users to appointments
  • Helping them with shopping
  • Assisting them with all aspects of care including personal care and medication management
  • Light household tasks e.g. preparing meals

You will be expected to work as part of the on-call team on a rota basis.

You will contribute to the delivery of care in accordance with the Health & Social Care Act 2008. You will take the lead in providing personal care, practical & emotional support to our service users in their own homes, giving them the ability to live safely in their own homes and live the life they choose.

Our service users are aged 18+ with a wide range of conditions under the following headings:

  • Dementia

All that we require from you is that you have experience & knowledge caring for people with a variety of conditions, ideally having an NVQ or diploma Level 3 in Health & Social Care. You will need to possess great management skills, with good written/verbal communication. We prefer for you to have a FULL DRIVING LICENSE or be able to get around Milton Keynes easily.
