Occupational Therapist Care Manager

Sirona Medical Social Work department is currently seeking a qualified and motivated Occupational Therapist Care Manager to join the Community Independence Service (CIS) Reablement Team on an agency basis.

Position Overview
We are currently looking for a qualified and motivated Occupational Therapist Care Manager to join our agency at Sirona Medical Social Work department. In this role, you will work as part of a multidisciplinary team, including Occupational Therapists, Independent Living Assessors (ILAs), and Social Workers, and will collaborate with health professionals and Reablement care providers. This is a frontline role supporting residents to regain independence following hospital discharge or health crises.

Key Responsibilities

1. Deliver skilled Occupational Therapy interventions to residents in their homes.
2. Manage cases for up to six weeks with a goal-focused reablement plan.
3. Conduct holistic assessments to identify clinical and personal needs.
4. Develop and implement intervention plans, including complex movement and handling assessments.
5. Order and provide adaptive equipment to support independent living.
6. Work collaboratively with a multidisciplinary team to ensure effective service delivery.
7. Maintain accurate case records using the Mosaic system.

Qualifications

1. BA Honors or Master’s Degree in Occupational Therapy.
2. HCPC Registration.
3. Eligibility to work in the UK.
4. Experience in adult social care or community settings.
5. Experience in complex movement and handling assessments and care planning (training and supervision available).
6. Strong case management skills, with experience in goal setting and strength-based interventions.
7. Ability to work effectively in a fast-paced, multidisciplinary team.
8. IT proficiency, with experience in Mosaic preferred.
