Job overview

Herts & West Essex Integrated Care Board (HWE ICB) is looking for an experienced Nurse who has specialised knowledge and experience of working with individuals with complex care needs. You will have worked in either a specialised or community setting with experience of the Continuing Healthcare process.

Main duties of the job

The post holder will be responsible for validating applications received from a variety of practitioners recommending people for Continuing Healthcare and NHS funded Nursing Care and act as a link person for continuing care service users and carers.

The post holder will be the case manager for people who are within the All Age Continuing Care [AACC] caseload and will provide professional advice to patients, carers and multi-agency colleagues.

The role will require a high level of communication and interpersonal skills and the ability to work supportively and effectively with individuals and their families ensuring that they are fully involved throughout the process.

Person specification

Qualifications

  • Registered Nurse (Adult or Mental Health) – with current registration
  • Educated to degree level in relevant subject or equivalent level qualification or significant experience of working at a similar level in specialist area
  • Post-graduate degree in Management Studies or equivalent

Experience and Knowledge

  • Experience of working in the NHS/public body
  • Evidence of working knowledge of CHC
  • Excellent understanding of the current issues impacting on the NHS, governance, policy and an experience of its implications so as to interpret and communicate them locally
  • Significant experience of successfully operating in a politically sensitive environment
  • Demonstrated experience of coordinating projects in complex and challenging environments
  • Experience of managing risks and reporting

Communication Skills

  • Ability to prepare and produce concise yet insightful communications for dissemination to senior stakeholders and a broad range of stakeholders as required
  • Experience of creating and giving presentations to a varied group of internal and external stakeholders

Analytical

  • Ability to analyse very complex issues where material is conflicting and drawn from multiple sources
  • Demonstrated capability to act upon incomplete information, using experience to make inferences and decision making

Planning Skills

  • Demonstrated capability to plan over short, medium and long-term timeframes and adjust plans and resource requirements accordingly
  • Experience of setting up and implementing internal processes and procedures.

Autonomy

  • Demonstrated capabilities to manage own workload and make informed decisions in the absence of required information, working to tight and often changing timescales
