Patient Pathway Coordinator, Neurology and Stroke

An exciting opportunity has arisen for someone to join the General and Specialist Medicine division at Ashford and St Peter's Hospitals NHS Foundation Trust. A 37.5 hour full time role of Outpatients Pathway Coordinator (Neurology and Stroke) is required to provide administrative support to the ASPH. It requires a high level of organisational skills and someone who is good at communicating with a wide range of health care professionals as well as patients and their relatives. The successful candidate will be responsible for many roles including organising timetables, dealing with clinic capacity and the outpatient waiting lists for both new and follow up patients. Ideally the successful candidate will have an understanding of the 18-week pathway.

Computer skills are essential, and training will be given on hospital administration systems.

Main duties of the job

You will work closely with Service Managers, Clinical Office Team Leader, Appointments Centre, Clinical Office Staff, Outpatient teams and Diagnostic and Therapy Departments across all specialties within the division to ensure patients are actively managed through patient pathways.

The role will involve monitoring patient's pathways from referral, through diagnostics, treatment planning and treatment, highlighting any potential delays in the pathway and ensuring accurate data collection to support the process and report to clinical offices any discrepancies.

You will need to ensure that appropriate requests for investigations are monitored following the clinician's instructions. If diagnostic delays occur or appear imminent, you will liaise with the relevant service for action.

You will liaise with members of individual specialties management teams and the Trust to ensure coordination of all elements of patient pathways. This may include highlighting patient target dates etc. to ensure that investigations are carried out and results received in a timely manner.

You will need to ensure all information relating to the patient's pathway clearly identifies all relevant target dates to aid other users of the relevant systems, such as Cerner, LUNA etc. in allocating resources appropriately.

Job responsibilities

To support the service managers in overseeing RTT 18 week performance management across the specialities within the MES division, providing expertise & support for the delivery & achievement of patient RTT standards.

Person Specification

Qualifications

  • English GCSE Grade C or equivalent
  • AMSPAR Medical Terminology

Experience

  • Excellent IT skills - confidence working with Microsoft Excel
  • Previous experience in an office environment working with health care professionals
  • Working to strict deadlines
  • Experience in using LUNA
  • Understanding of medical terminology

Knowledge

  • Understanding of GDPR and Informational Governance
  • Trust vision, values and strategic objectives

Skills

  • Able to communicate effectively to ensure high standards of care, treatment, service or support as appropriate to your role.
  • Able to provide a customer focused service.
