About the role

Are you a high-performing Adults' Social Worker looking to take the next step in your career? Join an innovative local authority that values potential and offers growth opportunities.

Our Adults’ Social Workers provide services across several areas, including Initial Assessment, Locality and Hospital Teams, and a specialist Learning Disability Service.

We currently have a Senior Social Worker vacancy in our Preparation for Adulthood Team, supporting young people transitioning to adult services.

Responsibilities:

  • Work collaboratively with the Transition Service, Children with Disabilities Team, Leaving Care Team, and SEN.
  • Conduct timely and responsive assessments, re-assessments, and reviews, delivering personalized care that supports independence and safety.
  • Manage cases, including EHCPs, health action planning, and person-centred planning.
  • Participate in Continuing Health Care (CHC) assessments when appropriate.
  • Support young people in their transition to employment.
  • Liaise with statutory, voluntary organizations, and other relevant agencies to ensure effective service delivery.
  • Represent the service in team and service developments, sharing information and knowledge.
  • Undertake Mental Capacity Act assessments, Best Interest and Community Deprivation of Liberty Orders.
  • Handle safeguarding referrals, risk assessments, and protection plans.
  • Provide duty and senior support and offer practice guidance within the wider LD teams.

About You:

  • You will be a qualified, experienced Senior Social Worker, ready to further your career within the adults’ Preparation for Adulthood Team.
  • You should have experience delivering statutory duties (Care Act/Safeguarding), ensuring timely assessments and smooth transitions for young people into adult services.
  • Creativity, innovation, and a commitment to exploring new models of working to improve outcomes for service users are essential.
  • You will be highly motivated and skilled in building strong relationships to deliver the best possible outcomes for vulnerable individuals.
