Base pay range

$18.00/hr - $20.00/hr

Position Summary: The Patient Care Coordinator plays a crucial role as one of the first points of contact patients have with the Practice. Acting as the central source of information, they provide clear, easily understandable details to patients about their upcoming consultations and assessments. The role requires creating a positive and supportive interaction that blends education with care, helping patients make informed decisions about their treatment options. Engaging in conversation with potential patients, the Coordinator helps them understand the benefits of our treatments, resolves any concerns regarding treatment, and ultimately creates a gold standard first impression with inquiries. During the initial consultation, the Coordinator balances gathering essential information with outlining necessary follow-up steps to ensure a smooth transition to the next phase of care. This is a fully remote position.

Key Responsibilities:

  • Initial Patient Consultations: Manage scheduling, rescheduling, and cancellations of initial patient phone consultations to ensure smooth operations and efficient use of resources.
  • Patient Communication: Respond promptly to consultation inquiries, appointment requests, and any questions, ensuring timely and professional communication.
  • Information Collection: Collect and document relevant patient information accurately during initial consultations to support effective care planning and follow-up.
  • Patient Account Management: Confidentially create, update, and manage patient accounts, ensuring accurate documentation and compliance with privacy standards.
  • Follow-Up: Complete follow-up activities on all initial patient consultations to ensure all necessary steps are taken for continued care and engagement.
  • Care Team Collaboration: Effectively communicate with other clinical support team members to ensure continuity of care and optimal patient outcomes.
  • Scheduling & Intake Process: Assist in completing the necessary steps for scheduling intake, including confirming availability, ensuring completion of intake forms, notifying clinicians, and entering appointments into the Electronic Health Record (EHR).
  • Patient Termination Process: Complete necessary steps and gather information during the patient termination process, ensuring proper documentation and follow-up.
  • Regular Patient Follow-Ups: Conduct regular follow-ups with current patients to monitor care progress and maintain engagement with both new and existing patients.
  • Team Collaboration: Work as an integral part of the team to provide excellent service and ensure that every patient receives the best care possible.
  • Regulatory Compliance: Ensure compliance with professional standards and regulatory requirements in all aspects of patient care, communication, and documentation.
  • Maintain Standards: Consistently enhance consultation outcomes and other key metrics to maintain and uphold the highest standards of care.
  • Community Resources & Referrals: Provide patients with community resources and referrals for continued care, supporting their ongoing health and wellness needs.
  • Special Projects: Engage in various projects, including outreach, process improvement, patient education, and clinician education, to support the broader goals of the practice and enhance patient care.

Work Location

  • Fully remote/virtual from employee’s site.
  • An employee is responsible for providing a private, confidential, and safe work space, with a professional background.
  • Employee is responsible for maintaining a fully operational and secure computer that can effectively operate necessary internet-reliant software (required software is provided).
