We are looking for someone with experience in healthcare, social work or education to come and join the team, leading on the personalised commissioning arrangements for people with Learning Disabilities, Autism, and Mental Health needs.
In the context of Learning Disabilities and Autism, this will include the SEL Learning Disabilities and Autism Programme. The post-holder is responsible for overseeing the processes around Care (Education) and Treatment Reviews (C(E)TRs) and the Dynamic Support Registers (DSR) for children and adults, improving the services available for people with Learning Disabilities and Autism in Southwark, those with mental health needs, and minimising the use of mental health hospitals for this group.
In a mental health context, this means supporting a select group of residents, with specific mental health needs to receive co-ordinated, personalised and holistic support to effectively meet their needs. This may include co-ordinating aspects of care & support with local and out of area providers. This role provides leadership and outcomes-focused management for commissioning for health and care services by building working relationships and collaboration across the council, the wider health system and through Partnership Southwark.
