This post attracts a welcome payment of 5% of the starting salary for the grade to enable individuals to join our permanent workforce. This is payable upon successful completion of your probationary period and is subject to eligibility criteria.

Are you passionate about making a difference in people's lives? Our Extra Care Team provides exceptional rehabilitation and support to adults, often frail and elderly, within their home environment. We aim to:

  1. Promote Independence: Help individuals live independently and prevent hospital admissions.
  2. Facilitate Hospital Discharge: Ensure smooth transitions from hospital to home.
  3. Maximize Community Living: Support independent living in the community.
  4. Assess Care Needs: Work closely with District Nursing, Social Care, and Housing Support colleagues to provide person-centered care.

Role: Stepdown Support Worker

As a Stepdown Support Worker, you will:

  1. Support Independence: Assist service users in living as independently as possible.
  2. Provide Personal Care: Offer basic health care and therapy, including mobility practice and exercise supervision.
  3. Collaborate with Professionals: Work with District Nursing, Social Care, and Housing colleagues to deliver comprehensive care.
Our Team

We are a friendly and supportive team, including:

  1. Registered Manager
  2. Care Manager
  3. Shift Leader
  4. Support Workers

You will be well-supported and work under the direction of a shift leader or care manager, helping service users achieve their re-ablement goals.

Position Details
  1. Schedule: Includes weekends and bank holidays on a rostered basis, with split shifts between 7am and 10pm.
