A specialist case management and rehabilitation company is looking for a Care Team Leader to join its team in Stockton on Tees. The company is committed to the continuous learning and development of all its staff to enable them to realize their potential.

This is a full-time role, working 37.5 hours per week.

As Care Team Leader, you will be working with one of the clients with Cerebral Palsy. Your responsibilities will include:

• Coordinating the rota to ensure cover is in place at all times.
• Ensuring the smooth running of operations and that everybody knows their tasks.
• Working on shift for the majority of your hours supporting with personal care, social and leisure activities, and rehabilitation activities.

You should have a great sense of humor, be kind, honest, and happy to support me in getting out and about and completing my therapies. I need support with my personal care and all aspects of daily living. I use a wheelchair to get around, although I can walk small distances with support, and I have my own wheelchair-accessible van. I'm very active and I like all things sensory, so you must be creative and be prepared to keep up with my changing activities. I live with my family who are very important to me.

We are keen to speak to people about this Care Team Leader opportunity who have the following skills and experience:

• Previous experience of working in the Health Sector.
• Trained to NVQ/Diploma Level 3 in Health and Social Care (or have equivalent experience e.g. completed year 1/level 4 of a recognized health profession degree course).
• Driving license and car ownership is essential.

This is a full-time position with afternoons, evenings, weekend shifts, annual leave/sickness cover available.
