Contract/Hours: Permanent / Full Time – 37 hours per week – Monday to Friday, 9am – 5pm however there will be a requirement of evening and weekend working

A bit about the role
Are you passionate about supporting young individuals in their journey towards independence? We're looking for a dedicated Transition Worker to assist young people transitioning from care to independent living or back home. This role involves collaborating closely with youths, their families, and various agencies to ensure tailored support is provided. Responsibilities include nurturing essential life skills, facilitating access to education and well-being services, and guiding them through the complexities of adulthood.
The AB56 service aims to provide holistic, needs-led, rights-based family support to families in Moray with children aged 0-12 and a child under 5 in the household. This Tier 2 Family support service bridges the gap between universal services and targeted interventions, reducing the need for referrals to Social Work services.
The service delivery model offers preventative interventions to families in their homes and communities, helping them identify needs and develop strategies to address them. The core theme of emotional, physical, and social wellbeing guides all service activities, with a focus on keeping children safe and placing families at the heart of decision-making processes.
