About the role:
You will join our valued team of Case Managers to support young people and adults following life-changing injuries. As a Case Manager, your focus is the client’s best interests first and foremost, coordinating care and rehabilitation tailored to their specific needs.
A Case Manager's aim is always to enable the client to regain and maintain the best quality of life by working closely with them, their family, and care team to support every aspect of their physical, cognitive, and emotional wellbeing.
The role is home-based, managing a client caseload with visits to your clients' homes within your geographical area. All ILS Case Managers are employed with the option for part-time salaried or flexible hourly employment, from 21 hours per week to 30 hours per week.
What is flexible hourly employment? You are paid for the work you do for our clients, including travel time and mentorship time. You need 3 days availability (about 21 hours) during Monday-Friday to carry out the work. It's a great choice to balance with other commitments.
You will ideally be a driver with a car as travel is up to 2 hours from your home to clients. Travel and mileage are paid.

About You
The Skills You'll be Sharing With The Team
You will be an experienced health and social care professional who has extensive expertise in working with young people and adults with catastrophic injuries, including traumatic brain injury, poly-trauma, spinal cord injury, limb loss, and cerebral palsy.
You will have experience of working directly with clients in the community and be able to demonstrate your professional abilities within the field of complex, long-term health care assessment, care planning, and achieving positive outcomes for clients.
You are able to work autonomously and in a proactive, solution-focused way using sound clinical reasoning in your actions.
You must have current HCPC/NMC/Social Work England registration and an up-to-date CPD portfolio.
