Job description

Permanent Contract | £27,711 to £32,654 per annum (Scale F/G) | 37 hours per week | County Hall, Norwich

Norfolk County Council has a shared vision ‘“To enable young people with learning disabilities, mental ill health, physical disabilities or autism, to live the lives they want by having the same access as their peers to opportunities for employment, housing, community inclusion and good health”. To help achieve this the Preparing for Adult Life (PfAL) Team has been set up as Norfolk’s transition to adult social services team.

We are looking for an enthusiastic PfAL Assistant Practitioner who will have the ability to work with young people from age 17 up to 20, as well as their families, be able to demonstrate an understanding of the legislative requirements and how to support young people to develop their independence. The role will involve carrying out assessments and care and support planning under the Care Act 2014.

You will also have a commitment to working in partnership with families as well as with the range of professionals and other agencies who could also be involved in supporting the young person as part of preparing for adult life.

Our Supported Assistant Practitioners (Scale F) join our 9-month, bespoke support programme, enabling you to progress to an Assistant Practitioner role (Scale G) subject to satisfactory programme completion.
