Hours: Part time - Up to 3 days per week

Salary: Dependent on experience

Ventnor Medical Practice is seeking an Advanced Clinical Practitioner/Paramedic/Advanced Nurse Practitioner to join our team as an important clinical member of the practice. This would be an opportunity to develop new clinical skills in a supportive and well-supervised environment.

Main duties of the job

  1. Work as a key member of the multi-disciplinary team to provide safe and high-level first contact care.
  2. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice.
  3. Implement and evaluate individual specialised treatment plans for chronic disease patients.
  4. Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects to the patient's health.

  10. Implement and evaluate individual specialised treatment plans for chronic disease patients.
  11. Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects to the patient's health.
  12. Be a professional role model for the organisation.
  13. Actively engage in peer review to inform own and others' practice.
  14. Critically assess and address own learning needs, negotiating a personal development plan that reflects the breadth of ongoing professional development.
  15. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
  16. Maintain accurate clinical records in conjunction with extant legislation.
  17. Develop, implement, and embed an effective call/recall system.
  18. Critically appraise and synthesise the outcome of relevant research, evaluation, and audit.
  19. Develop and implement robust governance systems and contribute to practice targets.
  20. Chaperone patients where necessary.
  21. Support the team in dealing with clinical emergencies.
  22. Support the team with all safeguarding matters in accordance with local and national policies.
  23. Deliver opportunistic health promotion where appropriate.

Person Specification

Skills and Knowledge

Essential

  • Clinical knowledge and skills.
  • Understanding of evidence-based practice.
  • Broad knowledge of clinical governance.
  • Ability to record accurate clinical notes.
  • Ability to work within own scope of practice and understand when to refer to GPs.
  • Knowledge of health promotion strategies.
  • Polite, confident, flexible, and cooperative.
  • Motivated, forward thinker.
  • Problem solver with the ability to process information accurately and effectively.

Qualifications

Essential

  • Qualified healthcare professional.
  • Educated to Masters degree as detailed within the HEE document.
  • Registered on their appropriate professional register.

Other requirements

Essential

  • Enhanced Disclosure Barring Service (DBS) check.
  • Occupational health clearance.
  • Evidence of continuing professional development (CPD) commensurate with the role.
  • Access to own transport and ability to travel across locality on a regular basis.
  • Flexibility to work outside core office hours.

Experience

Essential

  • Experience of practice within the four pillars.
