We are an integrated hospital and community services healthcare organisation serving around 400,000 people in Hartlepool, Stockton and parts of County Durham. We also provide bowel and breast screening services and a number of community based services to a wider population in Teesside and Durham.
Our turnover is around £365m and we employ a number of medical, nursing, clinical and non-clinical support staff totalling approximately 5,500 people.
We operate our services from two main hospital sites, University Hospital of North Tees and University Hospital of Hartlepool.
The University Hospital of North Tees provides emergency and planned medical and surgical care, maternity services and a wide range of diagnostic services and outpatient clinics.
The University Hospital of Hartlepool provides patients with a wide range of diagnostic services and outpatient clinics, day case and low risk surgery.
We also run services from various other sites across the local area including Lawson Street (Stockton), One Life Centre (Hartlepool) and Peterlee Community Hospital.

Cardiac Rehabilitation Nurse - Based in North Tees and Hartlepool community working across the area.
Supporting patients post cardiac event, providing education and guidance on lifestyle behaviours. Leading on and participating in Cardiac Rehabilitation Exercise classes and carrying out required exercise assessments.

The post holder will be a nurse with cardiology experience who holds or is working towards clinical assessment skills and non-medical prescribing qualifications. Developing knowledge and skills of managing Cardiac Rehabilitation case load. They will work within a nurse-led team, contributing towards the provision of high-quality care to cardiology patients.
The main responsibilities of the role will relate to clinical expertise with an emphasis on initiating, performing and maintaining standards of clinical excellence. The post holder will be able to work within the multidisciplinary team. This will include the assessment, diagnosis, interpretation of results, management and review of cardiac patients, under the supervision of a cardiologist and lead/cardiac rehabilitation specialist nurse.
Most clinics are nurse-led and include the management of secondary prevention patients, Angina management, the post holder may also develop their skills to work within the nurse-led, Secondary Prevention Optimisation Clinics (PCI review clinics), Angina Management, assessing for commencement to cardiac rehabilitation exercise component, Health Education and Behavioural change, audit and research.
