Job Details Job Location: Codman Square Health Center - Dorchester, MA Salary Range: Undisclosed

Position Summary:

Codman Square Health Center vision is to be our community’s first choice for comprehensive, holistic, and integrated services and empowers individuals to lead healthy lives and build thriving communities. Primary Care Providers (Providers) are integral in achieving this vision by providing high quality coordinated clinical care, being leaders on care teams, and sustaining the health center through billable visits. The Provider is responsible for managing their panel of patients through direct patient care and supporting population health management with an integrated care team. The Provider is a participant and leader in health center improvement work, and supports practice transformation including transformation into a Patient Centered Medical Home.

Primary Responsibilities:

Clinical Care Responsibilities

  • The Provider will practice medicine consistent with established clinical standards.
  • The Provider will follow the Health Center clinical quality guidelines and will make satisfactory efforts to achieve clinical performance targets established by the Health Center.
  • The Provider is responsible for team-based management of all patients on their panel, including direct patient care and follow-up for all patients seen during regularly scheduled clinical sessions.
  • The Provider will complete documentation and billing in a timely fashion as described in the Health Center Documentation and Billing protocols.
  • The Provider will make satisfactory effort to remain up to date on coding rules and accurately and thoroughly code all care provided.
  • The Provider will make satisfactory effort to meet or exceed the productivity goals determined by leadership.

Administrative Responsibilities:

  • Providers are required to participate in meetings, trainings and performance improvement activities as determined by Medical Leadership.
  • Additional departmental responsibilities include but are not limited to: coverage for absent Providers, involvement in staff evaluations, and other reasonable tasks determined by the Chief Medical Officer, Medical Director or Practice Manager.
  • Providers are encouraged to participate in other community activities.

Professional Responsibilities:

  • Providers will conduct themselves in a professional manner in all formal patient, staff, and community interactions, including being punctual to all clinical sessions and meetings.
  • Providers must work together with medical leadership to meet the clinical obligations of the health center, including clinical coverage (e.g. On Call system and coverage for Holidays, primary care, urgent care, precepting) as determined by the CMO or delegate.
  • Providers must be familiar with and abide by Codman Square Health Center personnel policies, procedures and guidelines, and clinical protocols.
  • Partnering with health center administration, Providers will complete all compliance and licensing requirements by the deadlines outlined by Medical Leadership. Requirements include, but are not limited to: licensing, peer review, coding audits, and competency assessments.
  • Partnering with health center administration, Providers are required to maintain licensure and other privileges required to practice medicine in Massachusetts, participate in regular continuing medical education activities; cooperate with the Codman Square Health Center staff in maintaining privileges in all hospitals and managed care plans used by the Health Center.
  • Providers are expected to participate in professional development through engaging in evaluation and peer review processes.
  • When appropriate, Providers will supervise Physician Assistants and Nurse Practitioners.
  • Providers are expected to support the formalized educational endeavors and partnerships of the health center.

Practice Transformation / Performance Improvement Responsibilities:

  • The Provider will practice in a style consistent with the Health Center’s goal to provide patient-centered and team-based care, including sharing responsibility for care with the clinical team.
  • The Provider is expected to participate in and lead improvement projects, including transforming their personal practice to support health center initiatives.
  • The Provider will participate in team-based care, including team meetings and huddles to support population management, complex case management.

Qualifications:

  • Current Massachusetts Medical License
  • Board Eligibility/Certification in primary care specialty
  • Current DEA and Massachusetts Control Substance License
  • Experience using Epic EHR preferred
  • Haitian Kreyol, French or Spanish language skills preferred
