This position is eligible for a $2,500.00 retention bonus.

Job Summary:

The Providence Center Case Manager Thurbers Avenue Health Home will coordinate services for consumers to minimize stressors and to facilitate the reduction of psychiatric and substance abuse disorder symptoms. Encourage consumers to attain the highest possible levels of independence.

Duties and Responsibilities:

  1. Coordinate a variety of services.
  2. Make assessments and provide appropriate crisis interventions and risk management as needed.
  3. Teach consumers adult daily living skills.
  4. Initiate treatment and advocate for consumer within network of care.
  5. Negotiate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves.
  6. Transport consumers in own vehicle or Center van to planned activities and appointments.
  7. Complete recordkeeping in accordance with Health Information Services (HIS) and other center requirements within required time frames and abide by center confidentiality policy.
  8. Maintain cooperative relationships with Center staff, consumers, community agencies and the public.
  9. Attend trainings, case presentations and conferences.
  10. Other duties may be assigned.

Requirements: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.   1. Bachelor's degree (B.A.) from four-year college or university in Human Services. Experience in Human Services Field preferred.
  2. Must have valid driver's license and insured auto.
  3. Ability to define problems, collect data, establish facts, and draw valid conclusions.
  4. Must be able to be flexible.
