Are you a proactive Nurse or Paramedic with a passion for community and frailty care? Looking for a flexible role that offers fantastic development opportunities? Join our Community Treatment Team (CTT) on a bank contract, with the potential for block bookings.

About the Role

As part of the Community Treatment Team, you’ll play a key role in delivering urgent care in the community, helping to keep patients out of hospital and supporting them in their own homes. This is a dynamic, fast-paced role ideal for someone with experience in emergency care, urgent care, or primary care.

You will:
Assess and manage acutely unwell and frail patients in the community
Provide urgent care for minor injuries, minor illnesses, wound care, catheter care, and more
Work autonomously while being part of a supportive multidisciplinary team
Liaise with GPs, ambulance services, and community teams to provide integrated care

What We’re Looking For

NMC/HCPC registered Nurse or Paramedic
Experience in Emergency Care / Urgent Care / GP settings preferred
A proactive, keen-to-develop clinician with a strong interest in frailty and community care
Ability to work independently and as part of a team

Development & Progression

Main duties of the job

Due to the crisis nature of the role the practitioner will be supported to extend their skills in order to recognise a deteriorating patient. The practitioner will be supported with continuing professional development. Close teamwork is paramount in order to manage complex patients in the rapid environment.

1. To manage a specific specialised caseload of clients within the community, using a model of assessment, planning, implementation, and evaluation.
2. To provide professional and clinical support to staff working within CTT and collaborative services such as Acute hospital trust, GPs, NHS 111, and Ambulance Services.
3. To participate in the development of a high-quality service clinical governance framework.
4. Assist with the day-to-day management of CTT and co-ordination of workload and development of the team.
5. To participate in the implementation of NELFT & NEL service developments impacting on Community Services.
6. Establish and maintain effective communication and relationships across all disciplines and services.
7. To act as an autonomous practitioner to assess, plan, implement, review, and evaluate care, including complex care assessments.
8. To act as a positive role model and resource for all staff disseminating clinical skills and knowledge.

Person specification

Qualification

• First level Registered Nurse currently registered with the Nursing & midwifery Council/BSc (Hons) Paramedic Science/Dip HE in Paramedic Science registered with the HCPC
• Practice educator or other teaching qualification or equivalent experience

Experience

• Previous experience as a RN/Paramedic in a band 6 post or equivalent, working as an autonomous practitioner for at least 1 year
• Portfolio evidence of professional development and commitment to CPD and commitment to ongoing training, education and mentoring to develop clinical skills.
• Knowledge and skills to assess patients with complex presentations to arrive at a clinical decision.
• Excellent communication skills including influencing, negotiation and empathic.
