Are you a proactive Nurse or Paramedic with a passion for community and frailty care? Looking for a flexible role that offers fantastic development opportunities? Join our Community Treatment Team (CTT) on a bank contract, with the potential for block bookings.
As part of the Community Treatment Team, you’ll play a key role in delivering urgent care in the community, helping to keep patients out of hospital and supporting them in their own homes. This is a dynamic, fast-paced role ideal for someone with experience in emergency care, urgent care, or primary care.
You will:
Assess and manage acutely unwell and frail patients in the community
Provide urgent care for minor injuries, minor illnesses, wound care, catheter care, and more
Work autonomously while being part of a supportive multidisciplinary team
Liaise with GPs, ambulance services, and community teams to provide integrated care
NMC/HCPC registered Nurse or Paramedic
Experience in Emergency Care / Urgent Care / GP settings preferred
A proactive, keen-to-develop clinician with a strong interest in frailty and community care
Ability to work independently and as part of a team
