Job Summary
An exciting opportunity has arisen to join our friendly and dynamic team at Homerton Healthcare NHS Trust as a Tissue Viability Clinical Nurse Specialist. We are looking for someone who is passionate about Tissue Viability, has previous experience in working as a Tissue Viability Nurse, is able to work autonomously, and would like to develop their skills at band 7 level. The service provides hospital and community services; therefore, being able to develop effective working relationships across an extensive range of different health care professional groups is essential. The successful candidate will have experience of working in an acute setting and a good understanding of inpatient systems and processes.

The successful candidate will have experience of managing staff, incidents, staff rotas, and supporting others to develop, creating an environment of shared learning and reflection. You will be supported by the Tissue Viability Lead Nurse and the rest of the specialist team to develop your knowledge and skills, and will play a key role in developing the service, implementing evidence-based practice across the Trust. This role will directly manage junior staff within the specialist team to deliver high-quality care.

Main Duties Include

  • Independently provides holistic, pressure ulcer risk assessment, leg ulcer assessment, and wound assessments for individuals with health impairment, facilitating the identification of adverse conditions that impair tissue health.
  • Respect the individuality, values, cultural and religious diversity of clients and contribute to the provision of the service sensitive to these needs.
  • Promote good working relationships within the neighbourhood multiprofessional teams providing appropriate specialist advice and support.
  • Plan, deliver, and evaluate research-based programmes of care for patients referred to the Tissue Viability Service in conjunction with the referrer.
  • Work effectively as an independent, autonomous, specialist practitioner, meeting agreed targets and objectives.
  • Act as a change agent to disseminate and implement sound research evidence into existing practice which facilitates a proactive/preventative ethos of care.
  • Deliver direct highly skilled individualised patient care based on the principles of best practice.

Qualifications

  • Registered General Nurse (NMC Registered)
  • Mentorship and Preceptorship training
  • Evidence of ongoing specialist post-registration training
  • Leadership training

Skills / Abilities

  • Ability to assess and treat a variety of wounds (surgical, traumatic, pressure ulcer, leg ulcer, moisture lesions)
  • Ability to manage junior staff (including students), delegating appropriately, and to manage own and team workload effectively.
  • Good organisation and planning skills.
  • Ability to meet deadlines as required.

Experience

  • Experience of identifying and reporting safeguarding concerns
  • Experience of successfully managing conflict
  • Experience of contributing to service developments.
  • Experience of use of non-standard specialist pressure relief equipment

Knowledge

  • Knowledge/understanding of confidentiality issues
  • Understanding Mental Capacity Act 2005 and its implications for community nursing
  • Understanding confidentiality and its impact on professional practice
