Main duties of the job
Working remotely, our team triage information from colleagues and members of the public in the Wolverhampton locality to ensure that we refer and signpost people to the most appropriate service to meet their needs. In summary our Care Coordination work streams include:
In addition to the above, as a Junior Sister/Charge Nurse within the Care Coordination you will be required to provide managerial leadership support to the team, support staff in their development and lead with service innovation. Experience and confidence in utilising computers and web-based applications is essential.
