Description

Full/Part-time Performer Dentist required for 2-4 days per week in a busy, mixed, well-established, INDEPENDENTLY OWNED practice in Calne, Wiltshire.

08.30 – 16.30
2000-5000 UDAs available at £12 + per UDA depending on experience
Modern, fully computerised practice
iTero Scanner, Digital x-rays, Rotary endo
Excellent support team including a hygienist and two therapists on-site
On-site parking available
30 minutes from Bath, 35 minutes from Bristol and 30 minutes from Swindon
Available now, start date negotiable

Compliance Requirements

Full NHS Performer Number if applicable
Local Area Team confirmation
GDC certificate
UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within the last 5 years
Indemnity certificate
Passport
Enhanced DBS (within the last 12-36 months)
Clinical references covering places of employment over past 2 years (minimum 2 referees)
