Job Title: Social Worker - Preparation for Adulthood Team

Location: Uxbridge, London

Salary: £41,442 - £45,510 per annum

Job type: Full Time, Permanent

Hours: 36 Hours per week

About The Role

We are recruiting a high performing adults' social worker who is looking to develop their career in an innovative local authority committed to recognising potential.

Our Adults Social Workers provide services across several areas including Initial Assessment, Locality and Hospital Teams and a specialist Learning Disability Service.

We currently have a vacancy in our Preparation for Adulthood Team that supports young people transitioning to adult services.

The Post Holder Will Support In Responsibilities Include

  • To work as part of the Transition Service and with the Children with Disabilities Team, Leaving Care Team and SEN.
  • To provide timely and responsive assessments, re-assessments and/or reviews delivering personalised care to support users and carers, encouraging maximum independence and safety.
  • To provide case management, EHCP's, health action planning and person-centred planning.
  • To participate in Continuing Health Care (CHC) Assessments where appropriate. To support young people into employment.
  • To liaise effectively and work in partnership with statutory and voluntary organisations and any other relevant agencies.
  • To participate in team and service developments (representing the service as requested) disseminating information, sharing knowledge and undertaking Mental Capacity Act assessments, Best Interest and Community Deprivation of Liberty Orders.
  • Making referrals to supported living, safeguarding enquiries, carrying out best interest assessments for the client, risk assessments and protection plans.
About You

You will be a qualified, experienced Social Worker looking to progress your career with a wide range of experience working within the adults Preparation for Adulthood Team to continue to deliver against its statutory duties (Care Act/Safeguarding), to facilitate timely assessments and ensure a streamline transition to young people in transitioning to adults.

You will be innovative and creative with a willingness to explore new models of working to improve the outcomes for our service users. Moreover, you will be a highly motivated individual who is able to build strong relationships to deliver the best possible outcomes for vulnerable people.
