Job description

Hampshire and Isle of Wight Healthcare NHS Foundation Trust

We are recruiting for a District Nurse Clinical Specialist to come and join our team here on the beautiful Isle of Wight. Part time (30 hours per week) or Full time (37.5 hours per week) will be considered.

The District Nurse Clinical Specialist is a pivotal member of the Integrated Community Nursing Service, responsible for delivering and leading specialist clinical care for patients on the District Nursing caseload. In doing so, the post holder will be expected to assess, plan, deliver and evaluate patient centred programmes of care ensuring that they practice within their sphere of competence and knowledge.

You will have the flexibility and skill set, to work across planned pathways and with specialist services, supporting patients to live or die well in the place that they call home, and prevent unnecessary hospital admissions in collaboration with Virtual Ward.

The successful candidate will be required to undertake relevant training events at any site across the trust.

This post is required to work in any location across the three localities.

Main duties of the job

Duties will include:

  1. Lead and manage a team to deliver care in the home and community. Be responsible for developing new and innovative clinical practices, responding to emerging knowledge and techniques working with other services where appropriate to make improvements.
  2. Work in an integrated and partnership way within the Integrated Localities, primary, secondary, social care, the independent and voluntary sector, and others to improve the health and care of individuals, families, and communities, particularly the most vulnerable reducing health inequalities.
  3. Work to professional and regulatory body codes, standards and guidance at all times ensuring clinical practice is grounded in evidence based theoretical and practical knowledge.
  4. Work in partnership with the wider Community Services Division taking a lead with the quality and governance agenda.
  5. Act with autonomy and authority, making decisions within an agreed professional and managerial structure.
  6. Create and promote a learning culture within community nursing to support the professional growth and development of nursing staff, students and as appropriate members of the multi-disciplinary team.
  7. Provide information, education, support and advice to patients, families, carers and health care professionals about aspects of care relating to Community Nursing and Locality Therapy.

Person Specification
Qualifications
  • Registered with current NMC Registration
  • Hold a recognised Educational/ Supervisory qualification or equivalent experience
  • Specialist Practice Degree (SPQ) in District Nursing or equivalent Advanced Clinical Practice qualification
  • Educated to a minimum of honours degree Level and be willing to progress to Masters Level
Experience
  • Significant levels of specialist knowledge and experience, for example in long term conditions management, care of the elderly, Chronic Obstructive Pulmonary Disease, evidenced through short courses and/or continuous professional development.
  • Independent / supplementary prescriber or commitment to undertake this course
  • Have proven experience at leading and managing people across disciplines and teams
  • Demonstrate knowledge and skills in understanding and delivering change in service delivery
  • Have a proven track record and experience in managing Performance and Quality
  • Have highly developed knowledge of Health and Social Care organisations
  • Have expertise in service improvement
  • Be proficient with a range of IT systems
  • Be confident at presenting complex data and information
  • Demonstrate specialist clinical knowledge and expertise that promote Positive changes in practice
  • Maintain clinical competence
  • Knowledge of the Continuing Health Care (CHC) process/personal health budgets
  • Have highly developed knowledge and experience of Governance and Quality assurance processes
  • Have highly developed interpersonal communication skills
