Then come and join Nottingham CityCare Partnerships Community Nursing team where we currently have 6-month secondment for a forward thinking, dynamic and compassionate Specialist District Nurse Lead. Successful applicants will have excellent clinical, communication and leadership skills and be directly responsible for a complex caseload whilst providing specialist training, advice, mentorship teaching and support to nurses and their teams. This District Nurse Lead role will also support and address the needs of both the professional and the leadership agenda of community nursing services.

This Specialist District Nurse Lead role will:
Utilise highly developed specialised and advanced clinical knowledge covering a range of procedures and underpinned by relevant broad-based knowledge, experience, and competence as a qualified District Nurse.
Where appropriate to service delivery, demonstrate highly developed physical skills requiring accuracy and dexterity e.g, moving and handling techniques, clinical procedures, diagnostic tests.
Demonstrate independent practice responding to social, scientific, clinical, and ethical issues which are encountered.
Make complex judgements requiring analysis, interpretation and comparison of options by integrating complex knowledge sources in new and or unfamiliar contexts.
Responsible for complex clinical pathways.
Engage and actively involves the individual and, if appropriate, their family or carer, in the assessment, planning, implementation and evaluation of programmes of treatment and or care by using person-centred techniques to promote a culture of self-care and empowerment.
Work to standards of proficiency identified by their professional and regulatory bodies performing clinical treatments and procedures to a highly specialist standard.
Prioritise their own workload and that of the wider team specifically that of the band 6 District Nurses within agreed objectives deciding when to refer to others as appropriate.
Provide and receives complex, sensitive or contentious information where motivational, persuasive, empathetic, negotiating and reassurance skills are required whilst demonstrating an understanding of barriers to communication.
Approach each individual with care, compassion and sensitivity ensuring that these values are reflected in all aspects of the role including the management of complaints and compliments.
Comply with the organisations Infection Prevention and Control policies.
Provide a leadership style which is underpinned by strongly held values around equality, diversity inclusion and openness; effectively builds and maintains relationships with direct reports, management team and other key individuals across the organisation.
Ensure that a professional service and positive image for CityCare is always maintained, thereby acting as a role model to all staff.
Work collaboratively with Heads of Service and Team Leaders in the leadership and development of community services.
Work collaboratively with the Director Nursing and AHPs.
Ensure the effective and efficient use of resources e.g. staff, supplies within their own sphere of responsibility.
Demonstrate leadership and innovation in contexts which are unfamiliar, complex, and unpredictable that require solving problems involving many interacting factors.
Demonstrate the ability to delegate, organise and prioritise workload to ensure the safe delivery of the service including the management of referrals and discharges utilising locally agreed support mechanisms e.g. on call manager and making judgements in complex situations.
Demonstrate resilience when responding to challenge, change and complex or difficult situations.
Demonstrate clinical leadership, initiative and creativity in developing projects inspiring others to be positive in their contribution to continuous improvement.
Clearly articulate the expected clinical standards of the service, monitors and identifies where standards are not met and takes action to address.
Ensure that all objectives, for self and those directly responsible for, are clearly defined within the wider CityCare structures and in line with CityCare objectives, using the appraisal process as a vehicle for this.
Take responsibility for their own and others health and safety in the working environment.
Undertake specialist training required to develop and maintain their own and others proficiency in response to changes in clinical practice and emerging techniques, demonstrating high levels of competence within professional body requirements.
Reflect on and evaluates own practice and identifies areas of development by setting appropriate objectives via appraisal and clinical supervision.
Initiate and develops research and development programmes appropriate to clinical practice.
Create and delivers specialist training and development programmes for Band 6 District Nurses, within teams and to other services, teams or agencies.
Provide high quality, specialist advice, support and supervision related to clinical specialism to Band 6 Nurses, other practitioners, agencies and individuals.
Ensure appraisals for direct reports are undertaken in accordance with Trust policy; manages performance which falls below standard in accordance with the appropriate HR policies and processes seeking support from senior staff where required.
Support the ongoing learning and development of others including students, preceptees and apprentices.
Is an active mentor assessor for student nurses and an advocate for increasing student learner numbers in practice.
