Hours: 37.5 hours per week (The service operates 8am – 6pm, 7 days a week; this post includes bank holidays and weekend working as part of the clinical team rota. There will be an expectation of some cross-site working as required.)

Shropshire Community Health Trust is offering an exciting opportunity for motivated individuals to work within the Integrated Health and Social Care Discharge Team at the Shrewsbury and Telford Hospital Trust. The team is expanding, and this is an ideal opportunity to be involved in the transformation of the service and work with the existing skilled team in the specialist field of Complex Discharge.

The successful applicants will be key members of the multidisciplinary health and social care Discharge Team.

Main duties of the job

The Discharge Support Worker will be instrumental in the support of identification, assessment and discharge of patients with complex needs from the hospital setting, agreeing the discharge pathway and services including home and bed-based reablement services, urgent community response (UCR) services including admission avoidance /virtual wards and community teams. The post holder will support co-ordination and navigate the patient across pathways to ensure timely discharge for patients deemed fit for discharge.

The post holders will work within the Care Transfer Hub providing a community-focused approach to discharge planning and assessment, embracing the principles of a Discharge to Assess Model and providing a person-centred approach to care.

The role and service offer integration and partnership working with team managers, nurses, therapists, health and social care coordinators across both organisations to ensure a seamless service.

The successful applicant will be supported by the Community and Acute Hospital Service Managers in the Integrated Discharge teams service development, working with the team in clinical, health care and social care practice, providing knowledge, skills and experience of acute and community health care to our patients to achieve timely and safe discharge from hospital.

Person specification
Training and Qualifications
  • GCSE level Maths and English or equivalent.
  • Evidence of relevant short courses and in-service training e.g knowledge of specific patient conditions, patient centred care/therapy, acquired through training, experience.
Experience
  • Demonstrate experience of working within a health and social care environment.
  • Ability to communicate with members of the public and health and care providers.
  • Knowledge of when to seek advice and refer to a registered care professional.
  • Experience of working with multidisciplinary health and social care teams in hospital, community, social or third sector.
  • Demonstrable knowledge of Discharge planning best practice.
  • General IT skills in the use of Microsoft products and clinical IT systems (e.g. RiO, Word, Excel, Outlook, Publisher, Powerpoint etc).
Knowledge and skills
  • Knowledge of the Hospital and Community discharge pathways.
  • Ability to analyse administration systems, implement change as directed and evaluate effectiveness in order to promote quality services.
  • Able to prioritise and meet deadlines effectively.
