Diabetic Nurse

We are looking for a dedicated and experienced diabetic nurse to provide care to our patients.

We currently work with a Lead Diabetes GP, HCAs and phlebotomists and admin support to work with patients to improve their diabetic health and health outcomes.

Main duties of the job

1. Provide care of newly diagnosed diabetics
2. Provide annual reviews to all our diabetic patients
3. Provide regular monitoring review of patients struggling to control their diabetes.
4. Provide Care summaries to patients to allow them to manage their own health decisions
5. Audit of diabetic outcomes when required
6. Provide adhoc answers to questions and support from diabetic patients (through the appointments system)
7. Provide expert advice to the clinicians at the practice about diabetes
8. Keep up to date on changes regarding diabetic care at national and local level. Disseminate to the team new information.

About us

The Leys Health Centre has a list of 12,300 patients around 800 of which are diabetic. It has a fantastic team of 10 GPs, 4 nurses, 2 HCAs and a phlebotomist as well as a great support team one of which is dedicated to diabetes care.

We work closely with two other practices through the PCN which includes a pharmacist team who are able to provide pharmaceutical advice when needed.

Job responsibilities

1. Undertake consultation at an advanced level, using expert knowledge and clinical skills to ensure patients receive high quality care, delivered in a timely manner.
2. Clinically examine and assess patient needs from a physiological and psychological perspective and plan clinical care accordingly.
3. Assess, diagnose, plan, implement and evaluate interventions/treatments for patients with complex needs.
4. Prescribe and review medication for therapeutic effectiveness, appropriate to patients' needs and in accordance with evidence-based practice and national and practice protocols, and within the scope of practice.
5. Make direct referrals to secondary care and lifestyle services within locally agreed pathways.
6. Maintain accurate and contemporaneous records using the practice clinic system.
7. Request prescriptions to GPs and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
8. Run Diabetes Specialist Nurse Clinics within the practice to incorporate monitoring the controls of existing Diabetes and ensuring accurate diagnosis, care and education of all new Diabetic referrals.
9. Provide Diabetic patients with patient centred, evidence-based care and treatment according to local protocols, guidelines and publish data from recent major Diabetes trials.
10. Prevent and detect early complications and other physical, social or psychological problems.
11. Liase with other health professionals or voluntary organisations both community and hospital based.
12. Introduce and follow the practice of Insulin conversion, including frequent regular follow-up.
13. Provide a safe and comprehensive service for altering Insulin dosage or type, either with or without GP reference.
14. Encourage patients to self-adjust their own Insulin.
15. Participate in group consultations for diabetes.

Person Specification

Qualifications

• Diploma or equivalent in Diabetes Management
• Prescribing Qualification
• ANP Qualification

Experience

• Recent and relevant experience of working in a Diabetes role.
• Post registration experience (Minimum 5 years).
• Relevant experience of management of long-term conditions.
• Maintenance of up-to-date skills, knowledge and understanding; and ongoing competence in Diabetes.
• Evidence of capacity to work successfully, both as an autonomous clinician and as part of a team.
• Proven ability to evaluate the safety and effectiveness of others and own clinical practice.
• Interpreting and implementing local and National policies, agendas for Health.
• Experience of delivering education and training to colleagues.
• Experience of supporting and educating patients to manage their health needs.
