An exciting opportunity has arisen to cover maternity leave in the established Diabetes Nursing team!
This role will be for one year fixed term and predominately be based providing care for people within the community diabetes service. The role will be based within the Community but working across an integrated nursing service using experience and specialised knowledge to provide education and advice, initiation and adjustment to treatment plans.

Main duties of the job
The community diabetes specialist nurse will assist in the delivery of a specialist clinical diabetes service with an aim to provide high quality diabetes care within Swindon in line with the direction of the service and Trust objectives. They will also help to deliver high quality education to practice nurses and GPs to improve diabetes care within the primary care setting.

The Diabetes Specialist Nurse will be committed to promoting a greater quality of life, short and long term, for the individual with diabetes and their family.
