An exciting opportunity has arisen to cover maternity leave in the established Diabetes Nursing team!This role will be for one year fixed term and predominately be based providing care for people within the community diabetes service. The role will be based within the Community but working across an integrated nursing service using experience and specialised knowledge to provide education and advice, initiation and adjustment to treatment plans.

Main duties of the jobThe community diabetes specialist nurse will assist in the delivery of a specialist clinical diabetes service with an aim to provide high quality diabetes care within Swindon in line with the direction of the service and Trust objectives. They will also help to deliver high quality education to practice nurses and GPs to improve diabetes care within the primary care setting.

Person SpecificationQualificationsEssential- RGN
- 1st Degree Diabetes Nursing Qualification or equivalent
- Knowledge of research, Portfolio reflects a career long learning experience
DesirableExperienceEssential- Experience working with people with diabetes
- Experience in educating patients and Health Care professionals
- Experience of service development
- Experience running nurse led clinics
Desirable- Experience of working in a community setting
KnowledgeEssential- Ability to implement change
- Ability to develop services
- Wide Knowledge of developments within the diabetes field
- Ability to support other professional staff in developing skills
- Experience of multi-agency working
- Experience of working within a primary care setting and commissioning issues relating to diabetes
Desirable- Experience of teaching structured education
