We are looking for an experienced, motivated and committed Diabetes Specialist Nurse with a passion for improving the experience and clinical outcomes of our patients, and who inspires others to provide excellent patient care.
We are looking for a nurse who has experience of working autonomously, supporting patients with a variety of diabetes treatments, and playing a key role in education of other healthcare professionals.
The post holder will play a central role in the development and delivery of a high-quality specialist nursing service for patients with diabetes as part of the multi-disciplinary Diabetes Team at Salisbury Foundation Trust (SFT). The successful candidate will work autonomously and within the wider diabetes team, delivering timely support and education in specialist areas including pump therapy and associated technologies, preconception and antenatal care, young adult/transitional services as well as supporting other specialist areas such as renal and foot.
They will also have a key role in supporting other members of the immediate DSN team on a daily basis, providing clinical supervision, continual professional development support and upskilling where needed. It will be expected that education and support will be delivered to other health care professionals focusing on all aspects of diabetes. You will be expected to work across boundaries, collaborating with the SFT inpatient diabetes service, the primary care diabetes team, General Practitioners, and the wider multidisciplinary team.
As a specialist nurse, you will act as an accountable, autonomous practitioner in the coordination and delivery of evidence-based expert nursing care for diabetes patients, their relatives, and carers. You will use well-developed decision-making skills to formulate treatment plans and initiate changes in management of care when necessary.
