Job Overview

We have an exciting opportunity to join a team of dedicated Diabetes Specialist Nurses in the Elsie Bertram Diabetes Centre (EBDC), covering 3 teams within the trust.

At the EBDC, we have a firmly established multi-professional approach to care delivery with both Consultant and Specialist Nurses delivering clinics. We aim to educate and inform patients, their families, and carers for them to make choices about their individual health care needs and support them with self-management of their diabetes.

Main duties of the job

As a Diabetes Specialist Nurse, you will be involved in, following appropriate training, covering inpatient services, outpatient services and community clinics, including our specialist outreach clinics in antenatal and young adult services. A flexible approach will be welcomed and may involve some later clinic cover and weekends.

As part of your role, you will help to provide patient education on an individual basis, as well as part of a group setting, initiate insulin/GLP-1 therapy and latest technology used in diabetes care such as insulin pumps and CGM for people who need them. The post will involve ongoing review and support of our patients, their carers and/or family members with management of their diabetes in our specialist nurse-led and multidisciplinary clinics. There will also be opportunities to provide education for staff members of the wider MDT within the Trust.

Detailed Job Description And Main Responsibilities

  • Provide education and ongoing review/support to patients and/or their carers on self-management of their diabetes and their medication.
  • Be involved in helping support patients with the latest technology available in diabetes management, such as insulin pump therapy, CGM and Hybrid-closed loops.
  • Opportunity to work across all teams in the specialty, including inpatient, outpatient and in the community with an opportunity to work towards becoming a non-medical prescriber in future.
  • Implement Trust and Directorate evidence-based procedures, policies, protocols and guidelines.
  • Contribute to local policy making within the agreed Trust strategy.
  • Act as an ambassador for the Trust in professional and trust business settings.
  • Develop policies and procedures for nursing practice for patients with diabetes.
  • To belong to diabetes interest groups at local and national levels, undertake presentations and disseminate information gained to unit staff.
  • Manage own caseload of patients.
  • Work in accordance with Trust and Directorate Diabetes Management guidelines and help to develop these where necessary.
  • With the Senior Diabetes Nurse assess the training needs of nursing staff within the team and ensure these needs are met.
  • To provide ongoing advice, information and emotional support to clients with diabetes and their relatives and carers.
  • To assess, plan, deliver and evaluate educational needs and programmes for clients and their relatives/carers.
  • Initiate treatment for diabetes for those patients with poor glycaemic control under the guidance of the medical team, or nurse prescriber.
  • Give dose adjustment/titration advice for patients to be able to continue self-management, or to other prescribing healthcare staff.
  • Ensuring appropriate follow up for all diabetes patients.
  • Provide telephone support to patients/relatives/carers and other health care professionals including dose adjustment, sick day rules and hypo advice.
  • Initiate referrals to other health care professionals, e.g. podiatrist, dietician and community nursing teams.
  • To reduce perioperative delays in diabetes management.
  • Provide expert opinion/advice to other healthcare professionals within the Trust, either as part of individual patient review or wider Trust staff education.
  • Participation in nurse led clinics, both face-to-face and virtual clinics.
  • To be able to use all current and new technology to help with your clinical assessment of a patient.

Qualifications

  • Registered Nurse on the appropriate part of the NMC register.

Experience

  • Significant experience at Band 5.
  • Some experience in a position relevant to the speciality.

Skills

  • Good communication and interpersonal skills.

  • Teaching experience.

Travelling to work

Staff parking is available onsite; however, this is limited during core working hours. We have more capacity in the evening and at weekends and can usually provide spaces during those hours.

The NNUH is served by a number of public buses, and we offer generously discounted monthly passes with First Bus & Konect.

The Costessey Park and Ride runs regularly each day from 06:30 (from P&R) to 20:45 (from NNUH) and this service is free for NHS staff. There is a regular Thickthorn P&R service as well which is free. Applications to the Travel Office are required for this service. We also have a discounted service from the airport P&R if you purchase a bundle. Please see Konect website for further details.

The NNUH host multiple bicycle racks and a salary sacrifice scheme. We also benefit from a Beryl station onsite.

DBS
