The Community Diabetes Specialist Service is looking to recruit a qualified nurse with experience in Diabetes management for the post of Community Diabetes Specialist Nurse. The post holder will join the current team of Specialist Nurses, Educators, and administration staff. The aim of the service is to provide Specialist Diabetes support and interventions in the community and primary care. You must be a qualified nurse with at least 5 years post registration/graduate experience, and hold an accredited course in Diabetes, preferably a post Graduate Diploma in a Diabetes-related field. An ability to demonstrate experience and knowledge in education, teaching, and mentorship for healthcare professionals and patients is required. Nurse prescribing and experience of working in a Specialist Diabetes role is essential. Experience of carbohydrate counting, insulin pump management, and the delivery of structured education would be advantageous, although training will be provided for the right candidate where service delivery is required. Applicants must be able to work on their own as well as being part of the multi-disciplinary team. You will undertake nurse-led clinics, multidisciplinary clinics, home visits, and be a duty/triage Nurse. You will additionally play a key role in supporting and educating patients, as well as community and practice staff to improve the quality of Diabetes care.
